About this file
This file is a Subcontractor Summary Worksheet template prepared by the City of St. Petersburg, Florida, for use in competitive bidding processes. The worksheet requires prime contractors to disclose all intended subcontractors and document the percentage of work to be subcontracted for projects. Bidders must complete and submit this form as part of their bid package. The worksheet requires detailed information for each subcontractor, including legal name, primary business address, contact information, employee responsible for project supervision, tier classification (first or second tier only unless otherwise specified in contract documents), trade classifications, and subcontract amounts. The form also includes fields to track additions and deletions of subcontractors throughout the project lifecycle, with designated reason codes explaining changes.
Subcontractor changes are restricted after contract award and the pre-construction meeting and may only occur under seven specific circumstances: failure to execute a contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, refusal to perform the subcontract, substantially unsatisfactory work performance, or substantial project delays or disruption. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates compliance through alternative means. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department. The restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
