About this file
This is a Subcontractor Summary Spreadsheet instruction document issued by the City of St. Petersburg, Florida, establishing guidelines for prime contractors bidding on municipal projects. The document outlines the mandatory submission requirements for a completed Subcontractor Worksheet as part of all bid packages, specifying that contractors must disclose all intended subcontractors and provide detailed information including subcontractor legal names, business addresses, contact information, trade classifications, and subcontract amounts. The worksheet accommodates changes to the subcontractor list only under specific circumstances after contract award, including subcontractor failure to execute contracts, lack of appropriate licensing, bankruptcy or insolvency, eligibility issues, performance failures, work quality deficiencies, or substantial project delays. The instructions clarify that Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates compliance through alternative means, with restrictions on subcontracting percentages and multiple-tier subcontractors not applying to Federal-Aid Highway System or federally funded projects.
The worksheet requires prime contractors to identify themselves, designate the percentage of work permitted for subcontracting as specified in the contract documents, and list the name and date of the employee completing the form. Requests for subcontractor changes after contract award must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The document includes a detailed legend explaining seven specific justifiable reasons for subcontractor deletion and requires identification of whether each subcontractor is SBE or Minority Women Business Enterprise (MWBE) certified, classification as first or second tier only, and notation of additions or deletions with associated reason codes. The form totals all subcontract amounts and calculates the percentage against the overall contract amount to ensure compliance with subcontracting limitations established in the contract standards and bidding instructions.
