About this file

This is a Subcontractor Summary Spreadsheet worksheet from the City of St. Petersburg, Florida, designed to document all subcontractors proposed for a contract project. The worksheet must be completed and submitted with bid proposals and requires contractors to identify the prime contractor name, percentage of work to be subcontracted, and detailed information for each intended subcontractor including their legal name, primary business address, employee contact information for project supervision, trade classifications, and subcontract amounts. The document specifies that only first and second tier subcontractors are permitted unless otherwise noted in contract documents, and subcontractors cannot be changed or substituted after a prime contractor is awarded and the pre-construction meeting is held, except under seven specific circumstances: failure or refusal to execute a written contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform the work, failure to perform the subcontract, substantial unsatisfactory work performance, or substantial delays or disruptions to project progress.

Subcontractors designated as Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractors that are used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substituted, unless the prime contractor demonstrates SBE requirements are being met through alternative means. Requests for subcontractor changes must be submitted in writing to the City of St. Petersburg Procurement & Supply Management, Attention: Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet includes designated cells for procurement use only and contains a legend identifying reasons for adding or deleting subcontractors. Restrictions on subcontracting percentage and multiple-tier subcontractor use do not apply to FDOT LAP or federally funded projects. The related contract opportunity involves preventive maintenance and repair services for six Liebert (VERTIV eXM) Uninterrupted Power Supply Units located at the St. Petersburg Police Department Headquarters Building.
