About this file

This is a Subcontractor Summary Spreadsheet worksheet required by the City of St. Petersburg for a shelter bid project. The document serves as an instruction guide and template for prime contractors to disclose all intended subcontractors as part of their bid submission. The worksheet establishes that up to 80 percent of the contract work may be subcontracted. Prime contractors must complete the form with their company name, the name and date of the employee completing the form, and comprehensive details for each subcontractor including their legal name, primary business address, project supervision contact information, trade classifications, subcontract amounts, and tier level (first or second tier only). The form includes fields to track additions and deletions to the subcontractor list with corresponding reason codes and city approver sign-off.

Subcontractor changes are restricted after the prime contractor is awarded the project and the pre-construction meeting is held, except under specific circumstances such as failure to execute a contract, lack of appropriate licensure, bankruptcy, ineligibility, failure to perform, unsatisfactory work quality, or substantial project delays. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. All requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management Contracts Compliance Manager. The worksheet notes that restrictions on subcontracting percentage and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
