About this file
This is a Subcontractor Summary Spreadsheet instruction document prepared by the City of St. Petersburg, Florida, for use in conjunction with a crane and crane equipment inspection and repair services contract opportunity. The worksheet must be completed and submitted with all bids and establishes the framework for identifying and managing subcontractors on the project. Prime contractors must list all intended subcontractors with their complete legal names, primary business addresses, contact information, and the specific trade classifications and subcontract amounts for work to be performed. The document specifies that subcontractors may be categorized as either first or second tier, with restrictions on multiple-tier subcontracting unless otherwise noted in contract documents.
The spreadsheet establishes strict protocols for subcontractor changes after contract award, limiting substitutions to specific circumstances including contractor failure to execute agreements, lack of proper licensing, bankruptcy or insolvency, ineligibility, performance failures, unsatisfactory work quality, or project delays. Small Business Enterprise (SBE) subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates compliance through alternative means. Bidders who do not intend to use subcontractors must provide a written statement to that effect. All requests for subcontractor changes after award must be submitted in writing to the City's Procurement & Supply Management Department, specifically to the Contracts Compliance Manager. The document was last revised in January 2020 and applies to non-federally funded projects, as restrictions do not apply to FDOT LAP or federal projects.
