About this file

This is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida for use on contract projects. The worksheet is mandatory and must be submitted with all bids. Contractors must identify their company name, provide complete information for all intended subcontractors including their legal names, business addresses, contact information, and the specific trade classifications being subcontracted, and indicate whether subcontractors are Small Business Enterprises (SBE) or Minority/Women Business Enterprises (MWBE). The worksheet captures subcontract amounts, tier level (first or second tier only unless otherwise specified in contract documents), and tracks any additions or deletions of subcontractors after contract award. The form includes a requirement for the name and date of the prime contractor employee completing the worksheet and space for city approval documentation.

Subcontractor substitutions are restricted after the pre-construction meeting and may only occur under seven specific conditions: contractor failure to execute a written contract at the agreed price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantially unsatisfactory work performance, or substantial project delays or disruptions. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates alternative compliance. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. All information regarding subcontractor changes must reference the applicable reason code and receive city approval with the approver's signature and date.
