About this file

This is a Subcontractor Summary Worksheet instruction document for the City of St. Petersburg, Florida, establishing guidelines for the Neighborhood Crossings - FY23 project (Project No. 23073-100). The project involves roadway safety improvement construction at multiple locations throughout public streets and rights-of-way, including sidewalks, ADA curbs, ramps, signage, pavement markings, and related operations. Prime contractors must complete and submit the Subcontractor Worksheet with their bid, identifying all intended subcontractors and their corresponding work scopes and contract amounts. Subcontractors may not be changed or substituted after contract award and the pre-construction meeting, except under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, substantially unsatisfactory work, or substantial project delays or disruption. Requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management Contracts Compliance Manager.

The City of St. Petersburg permits up to 80 percent of the shelter bid contract value to be subcontracted. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substituted, unless the prime contractor demonstrates compliance through alternative means. Only first and second tier subcontractors are permitted unless the Contract Documents specify otherwise. The worksheet requires documentation of each subcontractor's legal name, primary business address, project supervision contact information, tier level, applicable trade classifications, and subcontract amount. Restrictions on subcontracting percentage and multiple-tier subcontractor use do not apply to FDOT LAP or federally funded projects.
