About this file
This is a Subcontractor Summary Worksheet instruction document issued by the City of St. Petersburg, Florida, outlining requirements for subcontractor disclosure and management on city projects. The worksheet mandates that prime contractors submit detailed information on all intended subcontractors at the time of bid submission, including subcontractor legal names, business addresses, contact information, trade classifications, and subcontract amounts. For the Faith Community Housing Assistance Consultant project specifically, up to 80% of contract work may be subcontracted. Subcontractors cannot be changed or substituted after contract award and the pre-construction meeting without documented justification, such as contractor failure to execute contracts, lack of appropriate licensing, bankruptcy, ineligibility, substantial underperformance, or project delay. Small Business Enterprises (SBEs) or Minority/Women-Owned Business Enterprises (MWBE) used to meet participation requirements must be replaced with similarly certified firms if substitution becomes necessary. Only first and second-tier subcontractors are permitted unless the contract documents specify otherwise.
The worksheet requires prime contractors to designate whether subcontractors are first or second-tier providers and whether they are SBE or MWBE certified. All requested changes to the subcontractor list must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org, with provisions for adding, deleting, or modifying subcontractor information using standardized reason codes. The document includes a legend of seven specific grounds for subcontractor replacement, tracking columns for city approval and dates, and instructions that the worksheet must be completed and submitted with the bid proposal. The worksheet template provides fields for calculating total subcontract amounts against the overall contract amount and the allowable subcontracting percentage.
