About this file
This is a Subcontractor Summary worksheet instruction document issued by the City of St. Petersburg for the repair and raising of the Maximo Boat Dock project located at 4801 37th Street South, St. Petersburg, Florida. The document establishes guidelines for prime contractors bidding on the project to disclose all intended subcontractors and provides a standardized template for reporting subcontractor information, including the subcontractor's legal name, primary business address, project supervision contact information, trade classifications, and subcontract amounts. The worksheet must be completed and submitted with all bids and serves as the official record for subcontractor utilization throughout project execution. The City sets a maximum allowable subcontracting percentage at 80 percent of the total contract value, with restrictions limiting subcontractors to first or second tier only unless otherwise specified in contract documents.
The worksheet establishes strict controls on subcontractor changes after contract award, permitting substitutions only for documented reasons including failure to execute contracts at specified prices, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantial unsatisfactory work performance, or substantial delays to project progress. All requests for subcontractor changes must be submitted in writing to the City's Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The form includes provisions requiring that any Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements be replaced with similarly certified subcontractors if substitution becomes necessary, unless the prime contractor demonstrates SBE requirements are being met through alternative means. The worksheet was revised in January 2020 and applies to all subcontracting arrangements for this City of St. Petersburg project.
