About this file

This is a Subcontractor Summary Worksheet for the City of St. Petersburg, Florida, related to the Repairing and Raising of Maximo Boat Dock project. The worksheet provides instructions and a template for prime contractors to disclose all intended subcontractors before bid submission. Bidders must complete and submit the subcontractor summary with their bids, listing each subcontractor's name, business address, project supervisor contact information, trade classification, subcontract amount, and tier level (first or second tier only). The maximum allowable subcontracting percentage for this shelter project is 80 percent of the total contract value. Subcontractors may not be substituted or changed after contract award and the pre-construction meeting without documented justification, including cases where a subcontractor fails to execute a written contract at the specified price, lacks appropriate licensure, becomes insolvent, is deemed ineligible, fails to perform, produces substantially unsatisfactory work, or substantially delays project progress.

The worksheet requires prime contractors to indicate whether each subcontractor is a Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractor. Any SBE subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. Subcontracting restrictions do not apply to FDOT LAP or federally funded projects. All requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager. The worksheet was last revised in January 2020 and serves as the official document for tracking subcontractor information throughout the project lifecycle.
