About this file
This is a Subcontractor Summary Worksheet from the City of St. Petersburg, Florida, designed to accompany bids for the Repairing and Raising of Maximo Boat Dock project. The worksheet must be completed and submitted with all bids and requires bidders to identify their company as the prime contractor, list all intended subcontractors with complete legal names and business addresses, and provide contact information for the employee responsible for project supervision. Bidders must specify whether subcontractors are Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) certified, identify the trade classifications being subcontracted, designate whether subcontractors are first or second tier, and provide subcontract amounts. If no subcontractors will be used, bidders must provide a written statement indicating this in lieu of listing subcontractors.
The worksheet establishes strict controls on subcontractor changes after contract award and the pre-construction meeting, permitting substitutions only under specific circumstances including contractor failure to execute, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, unsatisfactory work quality, or substantial project delays. Any SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates compliance through alternative means. Change requests must be submitted in writing to the Contracts Compliance Manager. The worksheet indicates that restrictions on subcontracting percentages and multi-tier subcontractors do not apply to FDOT LAP or federally funded projects, though no specific funding source or subcontracting percentage limits are noted in this particular opportunity.
