About this file

This is a Subcontractor Summary Worksheet instruction document issued by the City of St. Petersburg, Florida, applicable to a ballistic door installation project at the St. Petersburg Police Department Headquarters. The worksheet establishes the framework for identifying and documenting all intended subcontractors for the project, with a maximum of 80 percent of contract work permitted to be subcontracted. Prime contractors must complete and submit the Subcontractor Summary form at the time of bid, listing all subcontractors by name, business address, trade classification, and subcontract amount. The document specifies that subcontractors may not be changed or substituted following contract award and the pre-construction meeting except under seven defined circumstances: failure to execute a written contract at specified terms, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantially unsatisfactory work performance as determined by the engineer, or substantial delays or disruption to project progress. Requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org.

The worksheet requires that any Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet applicable participation requirements must be replaced with similarly certified subcontractors if substituted, unless the prime contractor demonstrates that requirements are being met through alternative means. Only first and second-tier subcontractors are permitted unless otherwise specified in contract documents. The form functions as a tracking mechanism for the City's procurement office and includes fields for approval, deletion codes, and dates. The instructions indicate that subcontracting restrictions do not apply to FDOT LAP or federally funded projects. While the document itself does not disclose specific budget or award information, it serves as a mandatory compliance document for all interested bidders on this City of St. Petersburg procurement.
