About this file

This is a Subcontractor Summary worksheet issued by the City of St. Petersburg, Florida for a sheltered contract opportunity related to Police Department Ballistic Door Installation. The document serves as an instruction template and submission requirement for prime contractors bidding on city projects. Bidders must complete and submit the worksheet with their bids, listing all intended subcontractors and their associated work scopes. The City of St. Petersburg permits up to 80 percent of the contract work to be subcontracted for shelter projects. Subcontractors must be identified at the time of bid submission, including their legal names, business addresses, project supervision contacts, business classifications, and subcontract amounts. The worksheet requires designation of whether subcontractors are first or second tier and whether they qualify as Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractors.

Once a prime contractor is awarded the project and the pre-construction meeting is held, subcontractors may not be changed or substituted except under specific circumstances including failure to execute contracts at agreed prices, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform the work, failure to perform the subcontract, substantially unsatisfactory work performance, or substantial delays to project progress. Any requested changes to the subcontractor list must be submitted in writing to the Procurement & Supply Management Department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. If an SBE subcontractor is replaced to meet SBE percentage requirements, the replacement must also be an SBE subcontractor unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The worksheet restrictions on subcontracting percentages and multiple-tier subcontractor use do not apply to FDOT LAP or federally funded projects.
