About this file

This is a Subcontractor Summary Spreadsheet worksheet requirement document issued by the City of St. Petersburg, Florida for use in conjunction with the Police Department Ballistic Door Installation project. The worksheet must be completed and submitted with all bids and serves to document all intended subcontractors and their respective scopes of work. Prime contractors are required to enter their company name, identify the responsible employee completing the form, and list each subcontractor's complete legal name, primary business address, project supervision contact information, trade classification, subcontract amount, and tier level (first or second tier only, unless otherwise specified in contract documents). The document specifies the percentage of contract work that may be subcontracted and requires contractors to indicate whether no subcontractors will be used by providing a written statement.

Subcontractor substitutions are prohibited after contract award and the pre-construction meeting, except under seven specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantially unsatisfactory work quality, or substantial project delays or disruption. Small Business Enterprise (SBE) and Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with SBE subcontractors if changes occur, unless the prime contractor demonstrates SBE requirements are met through alternative means. All requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management office. The form was last revised in January 2020 and notes that subcontracting restrictions do not apply to FDOT LAP or federally funded projects.
