About this file
This is a Subcontractor Summary Worksheet issued by the City of St. Petersburg for a fire sprinkler modification and halon system removal project. The worksheet serves as an instruction document and form template that prime contractors must complete and submit with their bids. The document outlines that up to 80 percent of the contract work may be subcontracted, and it requires bidders to provide comprehensive information on all intended subcontractors, including their legal names, business addresses, contact information, trade classifications, and subcontract amounts. Prime contractors must identify whether subcontractors are small business enterprises (SBEs) or minority/women business enterprises (MWBEs), designate whether they are first or second-tier subcontractors, and specify whether subcontractors are being added or deleted from the project scope. The worksheet includes a legend of seven specific reasons that permit subcontractor substitution after contract award, including failure to execute a written contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, refusal to perform, substantially unsatisfactory work performance, or substantial delay or disruption to project progress.
Subcontractor changes must be requested in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet emphasizes that any SBE subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The form includes a calculation section for total subcontract amounts and the percentage of work allowed to be subcontracted relative to the overall contract amount. Notably, the restrictions on subcontracting percentages and the use of multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
