Project Design Scope of Work

GENERAL INTENT: The Contractor shall provide construction services to meet the drawings and specifications provided as SOW attachments 1 and 2 respectively (Henceforth referred to as the design documents) .

STATEMENT OF WORK:

GENERAL

The scope of work of this project is to complete the work shown in the design documents. The contractor shall be responsible for providing all tools, equipment, trained personal, and materials in order to provide a completed project in accordance with the design document.

CONTRACTOR Shall Provide

• Project scheduling and phasing.to include a full phasing plan that shall be approved by the COR.

• The contractor shall use the supplied sole source list and shall also identify any other limited source requirements that may exist and bring them to the attention of the COR as soon as possible.

Additional Contract Items and Requirements ENVIRONMENTAL PROTECTION: The contractor shall ensure that all federal and state environmental polices are adhered to during construction, this shall include VA Butler MCP Attachment, project specifications, and all VA directives.

Safety: The contractor shall follow all requirements set forth within the specifications, and within the VA Butler MCP Attachment, the project specifications, OSHA requirements, and any other applicable safety requirements.

PHYSICAL SECURITY: The contractor shall abide by all physical security and key control requirements set forth in the MCP attachment and within the contract documents. All contractor employees who will be on site for over a total of five aggerate days shall be submitted for badging and suitability review in accordance with HSPD-12 and VA Directive 0710. Per the positioning designation tool, a tier 1 background investigation will be required for all employes on site over five aggerate days.

SUBMITTAL REQUIREMENTS: The contractor shall route all submittals to both the A/E and to the VA. All submittals shall meet the requirements set forth within the specifications and shall highlight all selected items and materials. Any submittals returned to the contractor for rework shall be at the contractors own time and expense. Submittal documents shall be prominently marked “Controlled Unclassified Information (CUI)” if they contain qualifying data as defined in VHA Memorandum 2019- 11-13 Sensitive Infrastructure Data Classification – Controlled Unclassified Information (CUI) (SOW Attachment 2). This memo is attached to this SOW. All submittals and project documentation shall occur in Butler VA’s Autodesk Construction Could platform, licenses will be provided by the government at no cost to the contractor.

SUBMITTAL FORMATS: Submittals shall be submitted in electronic format and hard copy formats as listed below. Portable Document Format (.pdf), Microsoft Office Suite formats (.doc, .xls, and .ppt), Microsoft Project (.mpp) and Autodesk Revit Project (.rvt) files are acceptable. The CONTRACTOR shall be

Meeting/Conference Minutes

VA Butler’s desire is that status meetings will be held at a two-week interval during the design phase.

This will be discussed at the initial Status Update Meeting to ensure it is mutually agreed to by the contractor, VA Station personnel and the Post-Award Contracting Officer/Contracting Specialist.

Agendas and other required documents for all subsequent Status Update Meetings will be provided to the Post-Award Contracting Officer/Contracting Specialist and COR a minimum of 24-hours in advance of the meeting. Meeting minutes for all formal meetings and conferences held with the VA shall be submitted within three (3) working days of the meeting and at a minimum shall include the following:

a. Project name, project number, Meeting Name

b. A summary of the meeting including all participants

c. A Submittal Log of all submissions updated for each meeting.

d. A RFI log that shall be updated for each meeting.

e. A current, updated list of Action Items and responsible party for each action listed.

f. List any actions, events or concerns that may potentially delay the completion date of the project. If any are present, the contractor shall notify the Post-Award CO/CS and COR prior to the meeting via email.

g. Two week look ahead schedule.

h. Documents in any of the electronic formats listed above.

Change Orders and Contract Disputes The contractor shall notify the project COR immediately of any potential change orders or contract disputes. This will ensure that all parties are both aware of the issue and that a solution can be found for the problem that will be mutually beneficial.

PROJECT SCHEDULE/PERIOD OF PERFORMANCE: The contractor shall abide by the period of performance listed in this SOW and shall provide a CPM schedule that confirms to the specifications to show this. If at any time the contractor feels that they will fail to meet the period of performance set by this SOW, they shall immediately inform the COR of this so that corrective actions may take place.

CONTRACT OVERSIGHT: All contracts for this project (shall be awarded by Network Contracting Activity VISN 4 – NCO-4); only a warranted VA Contracting Officer shall have authority to change the terms and conditions of a contract. These changes may involve the contract award amount, contract period of performance, issuance of stop work orders and changes to the statement of work.

A COR from the Butler VAMC Engineering Service shall also be designated for the construction project contract to monitor and manage the construction contractor’s performance under the construction project contract. This designation under the construction contract shall be considered equivalent to the industry standard of “Owner’s Representative” and does not supersede or eliminate the contractor’s responsibility for oversight or management of their personal. The contractor ultimately shall be responsible for any errors and omissions with the craftmanship discovered during and after the construction phase.
