About this file

This document is a DLA Form 1889 containing multiple Supplemental Quality Assurance Provisions (SQAPs) prescribed by eSOP 4155.05-001 and sponsored by J3. The form serves as a template and collection of quality assurance requirements for Defense Logistics Agency (DLA) Energy contracts. Multiple SQAPs are included covering different contract aspects, each with distinct revision dates and approval authorities.

The document specifies point of acceptance provisions for Free on Board (FOB) origin and destination contract items, with acceptance occurring at origin or destination respectively unless stated otherwise in the contract. Energy QAP E12 establishes these acceptance points. Energy QAP E21.01 addresses point of inspection requirements for bulk fuel, bulk FSII, bulk lube oil, posts, camps, and stations programs, with distinct inspection procedures for FOB origin versus destination items, including provisions for waterborne transport and preliminary inspections. Energy QAP E22 identifies government inspection offices and their geographic areas of responsibility across the Continental United States, Alaska, Hawaii, and international locations by office code (110, 120, 200, 300, 400). Energy QAP E35 establishes procedures for nonconforming supplies and services, defining deviations and waivers, with submission requirements to the Contracting Officer and associated inspection offices, including consideration limits ($500 minimum for deviation/waiver approval). Energy QAP E37 addresses source restriction and source inspection protocols, allowing the Contracting Officer to require single-source designation upon delivery of off-specification products and to change inspection points from destination to origin following quality problems or recurring issues. The instructions provide detailed guidance for developing QAPs/SQAPs for individual or multiple National Stock Numbers or Local Stock Numbers using the prescribed form format.

INSTRUCTIONS

a. Developing a QAP/SQAP unique to an individual National Stock Number (NSN) or a Local Stock Number (LSN):

(1) Develop the SQAP using DLA Form 1889, downloaded from the Official DLA Forms website:

https://www.dla.mil/Working-With-DLA/Forms/DLFormList/. For the initial release of a QAP/SQAP, leave the REVISIONS block blank. When revising QAP/SQAP, update the REVISIONS block as appropriate.

(2) Enter the title of the SQAP in the DESCRIPTION block. This title should be the clear reason the SQAP was created. For DLA Energy, the applicable document number should be included in this block, as DLA Energy QAPs are numbered per contract clauses, materials, and geographic regions. (See the appropriate procurement guidance prescription list.)

(3) QA Requirements should be typed in the area below the Revisions block.

(a) If more information is needed or images are required, add a continuation page and add text or image(s) below the revision block.

(b) If files need to be attached, use the Attachment button to select and attach necessary files.

(4) Select the MSC responsible for the NSN or LSN by selecting the respective name in the address block. This will auto populate Commercial and Government Entity Code (CAGE).

(5) Enter “Multiple NSNs or LSNs” in the ITEM block.

(6) Enter “Multiple NSNs or LSNs” in the NSN block.

(7) SIZE and SCALE blocks are automatically populated; do not change.

(8) For non-Energy MSCs, enter the document number of the QAP in the DRAWING NUMBER block. The entire document number MUST NOT EXCEED 21 characters (due to system constraints) and will include the acronym QAP or SQAP. Examples of multi-use QAP/SQAPs include QAP-020, QAP-E09, and SQAP-SER1.

(9) Specialists electronically sign and date the “Prepared By” block ONLY.

(10) DLA Weapons Support and DLA Troop Support, Team Leaders or First Line Supervisors electronically sign and date the “Reviewed by” and “Approved By” blocks. NOTE: DLA Troop Support Construction & Equipment (C&E) PS will forward the SQAP to another PS for review and signature.

DLA Energy personnel will follow guidance provided in the DLA Energy Procurement Instruction (DEPI).

(11) Initiate a new form for those revisions when or if content changes happen by following the instructions directly above.

(a) The REV block should be updated alphabetically, beginning with “A.”

(b) Select DATE from the dropdown calendar.

(c) APPROVED block should be the First Initial and Last Name of the individual making the changes to the SQAP.

REVISIONS

E37 SOURCE RESTRICTION AND SOURCE INSPECTION (PC&S)

(a) SOURCE RESTRICTION.

(1) If the Contractor delivers any product that is determined to be off-specification, the Contracting Officer (CO) shall thereafter have the right, upon giving written notice, to require the Contractor to designate a single source of supply for each destination set forth in the contract. Upon receiving such notice, the Contractor shall have 10 days to respond, in writing, after which time the source restriction will become effective.

(2) In addition to the name of a single source of supply, the Contractor must provide the address of the terminal or loading point to be used in drawing the requirements for each item in the contract.

(3) The Contractor may change suppliers only after requesting and receiving the express written approval of the CO.

(b) SOURCE INSPECTION.

(1) If serious quality problems arise or if a quality problem recurs, for which the Contractor was supposed to have taken corrective action, the CO shall, in addition to source restriction, have the right to change the inspection point from destination to origin (source) by advising the Contractor in writing. Source inspection will become effective 10 days after written notice has been received by the Contractor.

(2) At the time the change becomes effective --

(i) All appropriate contract provisions relating to origin inspection shall be incorporated into the contract for the item(s) involved;

(ii) Source restriction, if not already in force, will be invoked for all items involved, in accordance with (a) above; and

(iii) The cognizant DLA Energy field office, upon receiving written notification by the CO, will become the office responsible for inspection at the origin loading or filling point for providing necessary field assistance.

(c) Failure to provide the information requested or to take prompt corrective action may result in the item(s) and/or contract being terminated for default.
