About this file
This is a Contractor Past Performance Data Sheet template designed for DLA Disposition Services to evaluate contractor performance on previous contracts. The form is completed in two parts: Part I is submitted by the quoter and includes contract details such as the contract number, point of contact information, detailed work description, dollar value, subcontractors used, and any quality, delivery, or price problems encountered along with corrective actions taken. Part II is completed by the government or commercial respondent who directly worked with the contractor and returned directly to DLA Disposition Services (not to the contractor) to provide an objective assessment of past performance.
The evaluation criteria in Part II assess five key performance categories: technical performance and quality of product/service, management and key personnel adequacy, schedule compliance and timeliness, small business and socioeconomic utilization if applicable, and compliance with additional regulatory requirements such as environmental and EEO compliance. Respondents rate each criterion using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) with detailed definitions provided, and provide narrative explanations for each rating. The form concludes with an overall contractor performance rating and a yes/no question regarding willingness to award to the contractor again. This template is used as part of solicitation SP451026Q1021 for Solid Waste Removal Services at DLA Disposition Services Pearl Harbor facility, a small business set-aside contract with quotes due May 28, 2026.
