CAMP JAMES A GARFIELD JOINT MILITARY TRAINING CENTER

Project: CJAG Culvert Replacement 2025 Project No.: 3924007
Statement of Work
1 - General Camp James A. Garfield (CJAG) is a Joint Military Training Center comprised of 21,683 acres located at 8451 State Route 5, Ravenna, Ohio 44266. This project consists of the removal and replacement of repair of existing culverts to protect against erosion and scour as designated in the bid documents. The project will be broken down into a Base Bid and Options. The Base Bid work and Options are detailed in Section 4 – Work to Be Accomplished and Attachment 3 – Site Maps.
This project will be awarded as a Firm Fixed Price contract.
The recommended period of performance is 365 days from Notice to Proceed.
2 - Scope of Work The contractor shall furnish all necessary planning, design, labor, equipment, materials, supervision, inspection and testing to perform the work in strict accordance with this Request for Proposal, the current Ohio Department of Transportation Construction and Materials Specifications, and the current Unified Facilities Criteria. Work accomplished shall meet or exceed the specifications designated by the CJAG Department of Public Works.
3 - Site of Work Site of work is at various locations within Camp James A. Garfield Joint Military Training Center (CJAGJMTC), 8451 State Route 5, Ravenna, Ohio 44266. Areas of work are all within the post and are located on Attachment 3 – Site Plans.
4 - Work to Be Accomplished
All work shall be designed and constructed in accordance with the Ohio Department of Transportation Construction and Material Specifications, Ohio Department of Transportation Standard Drawings and United Facilities Criteria (UFC) 3-201-01 specifications. Work to be accomplished includes but is not limited to the following items.
BASE BID – DESIGN BUILD
A. Smalley Road Box Culvert
a. The existing culvert is a 6’ x 6’ concrete box culvert and is 32’ long. Rock gabion wingwalls have failed resulting in erosion of the roadway embankment. The existing culvert also appears to be undersized based on channel observations. Additionally, there is little clearance between the ends of the culvert and the edge of the pavement at each end, which creates a safety hazard for motorists.
b. Perform the necessary site investigations to include geotechnical, site survey, hydraulic analysis, wetland delineation, etc. to develop construction drawings and specifications required for a new box culvert. A prior recommendation for a box culvert with a 6’ rise and 12’ span and length of 40’ to include rock channel protection at inlet and outlet and new guardrail in compliance with design standards was made based on a recent bridge inspection survey.
B. Smalley Road – Area 1A Culvert
a. The existing culvert is two lines of 48” reinforced concrete pipe connected by a masonry catch inlet with a total approximate length of 162 feet. The existing catch inlet structure is failing and erosion is visible adjacent to the inlet creating a safety hazard to motorists and to mowing operators. Additionally, there is visible separation occurring at the pipe joints.
b. Perform the necessary site investigations to include geotechnical, site survey, hydraulic analysis, wetland delineation, etc. to develop construction drawings and specifications required for a new culvert that eliminates the catch inlet and reduces the overall length.
OPTIONS
C. OPTION 1: Pre-Construction Notification (PCN)
a. A PCN will be required. Bids will include costs to cover subcontracting with a qualified wetland consultant as well as PCN fees as part of the bid. Refer to General Wetland Requirements and Pre-Construction Notification (PCN) sections below.
D. OPTION 2: Erosion Repairs – Various Sites
a. NFR 2.52 – Place appropriately sized dump rock along east abutment to prevent further erosion and scour.
b. NFR 1.32 – Place appropriately sized dump rock at eroded areas at northwest and northeast quadrants.
c. GLR 2 - Place appropriately sized dump rock at inlet, outlet, and behind wingwalls along approach embankment at all four quadrants to prevent further erosion and eventual undermining of roadway pavement.
d. GLR 3 - Place appropriately sized dump rock at inlet, outlet, and behind wingwalls along approach embankment at all four quadrants to prevent further erosion and eventual undermining of roadway pavement.
e. Estimate a total of 100 tons of Type C Rip Rap to be distributed between the four sites above.
E. OPTION 3: Road 5C Culverts
a. Replace 18” diameter x 40’ long culvert. Install half-height headwalls at each end.
b. Replace 12” diameter x 30’ long culvert.
F. OPTION 4: Magazine Road Culverts
a. Replace 18” x 38’ culvert. Install flared ends each end. Remove 18” x 34’ culvert and catch basin and replace with swale.
b. Replace 18” x 52’ culvert. Install flared ends each end.
c. Replace 18” x 38’ culvert. Install flared ends each end. Remove 18” x 34’ culvert and (2) catch basins and replace with swale.
d. Replace 42” RCP x 54’ culvert. Remove catch basin and reroute roadside ditches culvert inlet. Install full-height headwalls each end.
G. OPTION 5: Randall Road Twin Culvert
a. Replace existing twin 30” x 42’ long culvert with half-height headwalls.
5 - General Requirements Examination of Contract Documents and Site:
Before submitting a Proposal, Contractor must: (a) examine Contract Documents thoroughly; (b) visit the site to familiarize himself with local conditions that may in any manner affect cost, progress of performance of the Work; (c) familiarize himself with federal, state, and local ordinances, rules and regulations that may in any manner effect cost, progress or performance of the Work; and (d) study and carefully correlate Contractor’s observations with the Proposal Documents. Each bidder is responsible for verifying all measurements prior to bid submittal. Request clarification via means of an RFI prior to submitting a proposal.
Field Inspection Visit:
1. Contractor shall thoroughly examine Contract Documents and shall thoroughly acquaint himself with the site and with the field conditions affecting the work.
2. The Contractor shall take his own measurements and be responsible for the same.
3. The Contractor shall verify conditions on the job so that all work will be properly coordinated.
Submittals:
Prior to the start of work, the Contractor shall submit for approval a project schedule, environmental work plan (EWP), and site supervisor resume. All materials to be used in the project shall be submitted and approved prior to installation. Refer to Attachment 4 - Schedule of Material Submittals for further information.
Requests for Information (RFIs):
Immediately on discovery of the need for information/interpretation of the Contract Documents, prepare and submit a formal Request for Information on the approved form that will be provided at the Preconstruction Meeting. Allow 10 (ten) working days for Governments Response.
Environmental:
The Contractor is required to comply with applicable portions of CJAG’s Environmental Procedures and Waste Management Guidelines (See Attachment 1).
General Wetland Requirements:
Culvert replacements must be done in accordance with the terms and conditions of U.S. Army Corps of Engineers (USACE) Nationwide Permit #3 (NWP#) and the associated Nationwide Permits Regional General Conditions, Corps NWP Specific Regional Conditions, Ohio 401 Certification Special Limitations and Conditions, and General Limitations and Conditions for all Ohio EPA Clean Water Act Sec 401 Certified Nationwide Permits. The Contractor will obtain a copy of the above listed permit conditions and will include them in their Environmental Work Plan. The Contractor will integrate permit conditions into their work process to ensure compliance.
When a Pre-Construction Notification (PCN) or Individual Sec 401 Water Quality Certification (WQC) is required or an evaluation is needed to determine if a WQC is required, the Contractor will use a qualified Wetland Consultant to do this work. The qualified Wetland Consultant will perform all work, except as noted in this section, necessary to develop a complete PCN to include a Sec 401 WQC evaluation and Individual Sec 401 WQC application if/as required. The PCN and/or WQC application must be reviewed and approved by the OHARNG prior to submittal.
PCN’s and WQC will be developed for signature by the OHARNG as the landowner. The Contractor will pay the cost for PCN and WQC development and submittal. Neither the Contractor nor the Contractor’s Wetland Consultant will contact the USACE or the Ohio EPA without written authorization from the OHARNG or the Adjutant General’s Department to do so.
One PCN may be developed and submitted for all awarded items. If a partial award is made and a later additional award is made, an additional PCN is required for any culverts not included in the initial award PCN. Project delays caused by Contractor failure to prioritize development and submittal of a PCN/WQC or the need to develop more than one PCN/WQC application will not be cause for a project extension or cost increase.
The OHARNG Environmental Department will provide the required ESA Sec 7 consultation and the NHPA Sec 106 consultation for the PCN. The Contractor is required to provide a project description and details necessary for the OHARNG to complete these.
Sec 401 Water Quality Certification (WQC) The Ohio EPA General Sec 401 Water Quality Certification is valid for NWP3 in all locations so a determination of eligibility using Ohio EPA Sec 401 WQC eligibility map is not required and an Individual Sec 401 WQC is not required.
Pre-Construction Notification (PCN)
1. A PCN is required for Base Bid, Task A; Base Bid, Task B; and all Options.
2. Using a qualified Wetland Consultant, the Contractor will prepare and submit PCN(s) in accordance with the General Wetland Requirements above.
3. The PCN(s) will be submitted to: U.S. Army Corps of Engineers, Pittsburgh District, William S. Moorhead Federal Building, 1000 Liberty Avenue, Pittsburgh, Pennsylvania 15222-4186, 412-395-7155.
Erosion and Sediment Control:
A construction storm water permit is not required for this project. The Contractor will install and maintain erosion and sediment control as necessary to prevent erosion from getting into streams and to prevent tracking of soil onto roads and from the work sites. Storm water controls must remain in place until a minimum of 70% of disturbed areas have a soil stabilizing grass cover established. Areas that are eroding along headwalls and slopes will be repaired and stabilized utilizing appropriate industry standard stabilization methods and as approved by the Environmental Office. Erosion and sediment controls and final site stabilization methodologies may require modification from what is in the EWP depending upon site conditions and time of year the work is completed. The Contractor is required to prevent erosion and sediment from entering streams and leaving the site and is required to stabilize and establish grass cover on the finished grade by whatever standard industry methodology is necessary regardless of whether the required methodologies are more intensive than what is identified in the approved EWP.
Waste Management:
The Contractor is required to divert recyclable materials generated during the project from going into a landfill. Recyclable steel will go into the CJAGJMTC scrap metal dumpster. Non-recyclable steel and all other materials will be removed from post and properly disposed of or recycled as applicable. The following must be removed from CJAGJMTC no later than the project completion date: any unused soil or organic spoils and debris, all steel, concrete, and wood or other construction materials, equipment, and vehicles.
Soil Spoil:
Excavated materials can be reused in the construction of the new culverts where the excavated material is suitable for these operations. In areas where the excavated material is not suitable for fill or backfill the Contractor shall supply suitable material that meets ODOT and Environmental specifications.
Staging Areas:
Staging areas for materials, equipment, and support operations shall be coordinated with CJAG Post Engineer.
Environmental Work Plan:
Prior to initiating field work, the Contractor will develop and gain approval of an abbreviated Environmental Work Plan (EWP). The EWP will be reviewed by the Environmental Office and revised by the Contractor to facilitate Environmental Office approval.
The EWP is only required to contain information applicable to work being performed. Do NOT include a copy of the Environmental Procedure as an attachment to the EWP. Applicable language and attachments from the Environmental Procedures may be incorporated into the EWP. Include written descriptions of the following at a minimum:
· General project description
· Site location descriptions with map showing the locations of the culverts
· Contractor and Government points of contact
· Description of work to be accomplished
· Number and description of culverts being replaced
· General work procedures
· How the sites will be finished-graded
· How compliance with USACE Nationwide Permit #3 will be obtained
· Stream protection measures
· How continual downstream flow will be maintained
· Length of riprap
· Vegetation clearing, if applicable
· Storm Water Management and Sediment and Erosion Controls
· Description of the site stabilization process
· Seed mix
· Spill Prevention, Response, Notification, and Reporting
· Emergency Plan and Points of Contact
· Waste Management
· Point of Contact
· Types of waste to landfill
· Types of waste to be recycled
· Waste management procedures
· Waste haulers (Company name, address, phone)
· Waste disposal and recycling facilities (Company name, address, phone)
· Include and use the OHARNG Construction/Demolition Diversion and Waste Disposal Form
Work Scheduling:
Normal base work hours for the Contractor will be between the hours of 7:30 am through 4:30 pm, excluding Saturdays, Sundays, and State Holidays. The normal work hours for National Guard personnel are 7:30 am to 4:30 pm, Monday through Friday. If the Contractor desires to work during periods other than above, Contractor must notify the Project Manager 5 working days in advance of Contractors intention to work during other periods to allow assignment of additional inspection forces and for notification of fire, security and safety. When the Project Manager determines that they are reasonable, Project Manager may authorize the Contractor to perform work during periods other than normal duty hours/days. However, if inspectors are required to perform in excess of their normal duty hours/days solely for the benefit of the Contractor, the actual cost of the inspection, at overtime rates, will be charged to the Contractor and will be deducted from the final payment of the Contract amount.
The following Federal/State legal Holidays are observed by this site:
1. New Year’s Day – 1 January
2. Martin Luther’s King’s Birthday – Third Monday of January
3. President’s Day – Third Monday of February
4. Memorial Day – Last Monday of May
5. Juneteenth – 19 June
6. Independence Day – 4 July
7. Labor Day – First Monday in September
8. Columbus Day – Second Monday in October
9. Veteran’s day – 12 November
10. Thanksgiving Day – Fourth Thursday in November
11. Christmas Day – 25 December Note: Any of the above Holidays falling on a Saturday will be observed the preceding Friday, holidays falling on a Sunday will be observed on the following Monday.
Prior to commencing work on the job initially, resumption of work after prolonged interruption (7 calendar days or more) commencement of any warranty work, and upon completion of warranty work, the Contractor must notify the Project Manager. Notification should be by personal contact; however, advance notification may be by telephone, or in writing, and should be accomplished sufficiently in advance to allow scheduling of inspection forces. The above precautions are to ensure construction inspection and recording of work proceedings.
Unauthorized Personnel:
The Contractor shall inform all personnel (including subcontractor and visiting supplier personnel) that access to areas outside of the immediate work area excluding, direct haul and access routes, and point of supply and storage, is prohibited. Circulation of said personnel will be limited to official business only.
Method of Carrying on the work:
All work under the Contract shall be arranged and carried on in such a manner as to complete work in the least possible time.
Activities in the vicinity of this Project may be kept in full or partial operation during construction. The Contractor shall coordinate with the Project Manager and schedule construction activities.
Misplaced Materials:
Any material that is deposited elsewhere than areas designated as approved by the Project Manager shall be rehandled and deposited where directed. No payment will be made for rehandling such material.
Inspections and Tests:
Inspections and tests are for the sole benefit of the Government and shall not relieve the Contractor of the responsibility of providing quality control measures to ensure that the work strictly complies with the Contract Requirements. No inspection or test by the Government shall be construed as constituting or implying acceptance.
Quality Control/Tests:
Where work is required to be in conformity with Standard Specifications of the American Society for Testing Materials (ASTM), or with State/Federal Specifications or with specifications of well-known recognized technical and trade organizations, but no tests are specifically stipulated in connection herewith, the Contractor shall furnish and pay for any tests or certification required by the Project Manager to show that the proposed materials meet with the applicable requirements.
The Contractor shall submit a written certification from a recognized testing agency, adequately equipped and competent to perform such services, that the material or equipment has been tested and conforms to the standards, including the methods of testing used.
Wherever testing or analysis of material is required, such testing unless otherwise noted will be made at the Contractor’s expense.
Tests shall be made in accordance with the specified testing procedures and/or methods and otherwise as required to provide compliance with all Contract requirements. Tests shall be made by independent, commercial testing laboratories approved in writing by the Project Manager.
Results of all tests shall be recorded on certified test reports of the commercial testing laboratories. Reports shall include a statement that the materials tested do or do not meet the requirements of the Contract Documents. Two copies of all reports shall be forwarded directly to the Project Manager for approval within 5 days of the actual performance of the test. The testing agency shall notify (verbally) the Project Manager of any tests, which indicated failure to meet the Contract requirements.
Any item, for which test reports show failure to meet all Contract requirements, shall be retested as often as required to show full compliance with Contract requirements, at the Contractor’s expense.
Contractor will provide an emergencies plan, with names of personnel to accomplish the repairs in the event of utility and/or communications emergencies.
Site Cleanup:
The Contractor shall maintain the construction site in as clean and orderly condition as possible. All refuse and/or salvage material shall be gathered and disposed of periodically to maintain the site in this condition. All roadways, and roadway areas within the work area, or used by the Contractor, shall be kept free of construction debris, and associated mud, dirt, sand, or gravel that may be tracked from the construction work area by cleaning daily to ensure safe operation of vehicles. The method of cleaning and equipment employed shall be subject to the approval of the Project Manager.
Following completion of the work, the Contractor shall clean the entire area from any debris and/or excess of misplaced material due to the Contractor’s operation and obtain the Project Manager’s approval of this finished work.
Storage:
No secure storage space will be provided by the Government. The Government will not be responsible for property belonging to, or under the present control of the Contractor. The Project Manager will designate an unsecured, open area for storage of construction equipment and materials during the period covered by this Contract.
The Contractor shall construct such temporary sheds, as may be required, for the use of workmen and as required for tool cribs and storage of all work under this Contract. Temporary sheds shall be confined to the space assigned by the Project Manager. Sheds shall be of approved construction and wood floors; lighting and heat shall be provided in all parts used by workmen. Exterior of sheds shall be painted, all parts maintained in good condition throughout the life of the Contract, and at completion, all parts shall be removed and the premises cleaned up.
Temporary Field Offices:
As soon as practicable after award of Contract, and until completion of the work, Contractor shall provide, maintain and later remove suitable temporary office(s). All field offices shall be in good repair, painted on the exterior, provided with adequate heating and lighting and be maintained in a clean and sanitary condition at all times.
The Contractor shall provide temporary office space for exclusive use of the Government inspectors, to include contracted inspectors. This office shall include, as a minimum, two desks, two suitable chairs, access to a phone line and separate high speed internet connection.
The Contractor is reminded that smoking in buildings on a government facility is prohibited. A smoking area for employees, away from the main entrance of a building must be provided.
Temporary Toilet Facilities:
The Contractor must provide all temporary toilet facilities. The temporary toilet facilities shall meet the requirements of health authorities having jurisdiction and shall always be kept clean and in a sanitary condition.
Utilities:
Government operated utilities will not be furnished to the Contractor without charge where existing outlets are available. The Contractor is responsible for installing temporary service outlets, as necessary. Any expense incurred to gain access to these utilities (temporary tap-ins, etc.) shall be the responsibility of the Contractor and all utilities shall be returned to their original configurations at the end of the contracts. No alterations to existing utilities shall be accomplished without the written permission of the Project Manager.
The Contractor’s progress schedule shall include a preliminary listing of all proposed shutdown dates. Every effort shall be made to make all shutdowns as brief as possible and as limited as possible.
Damages, Repairs:
All damages by the Contractor’s operations shall be repaired, or replaced at the Contractor’s expense, as directed by the Project Manager. Any Government property damaged because of the work, materials, or operations of the Contractor shall be restored at no additional expense to the Government.
All existing sidewalks, curbs, fencing, and pavement disturbed, broken or removed or otherwise damaged by the Contractor during performance of the work under this Contract shall be replaced by the Contractor at the Contractor’s own expense. Replaced sidewalks, curbs and pavements shall be smooth, shall blend into the existing work, and shall not present depressions or humps.
Maintenance of Traffic and Safety:
Where possible, construction work shall not interfere with the normal operations of traffic, particularly emergency vehicles and equipment. The contractor is responsible for safety on the Project Site.
The Contractor shall use only established haul routes. When materials are transported, vehicles shall not be loaded beyond the loading limit established by Federal, State, or Local Law or regulation. When it is necessary to cross curbing or sidewalks, protection against damage shall be provided by the Contractor.
The Contractor shall provide for the free and unobstructed movement of Military vehicles with respect to all operations and the operations of subcontractors.
With respect to the Contractor’s own operations, and those of all the Contractor’s subcontractors, the Contractor shall provide marking, lighting, and other acceptable means of identifying personnel, equipment, vehicles, storage areas, and any work or condition that may be hazardous to the operation of military vehicles, fire-rescue equipment, or maintenance vehicles at the Post.
The Contractor shall furnish, erect, and maintain weighted barricades, warning signs, and other traffic control devices as required to maintain traffic and ensure the safety of military vehicles in the area and the Contractor’s equipment. The Contractor shall make an estimate of all labor, materials, equipment, and coincidental necessary for providing the maintenance of vehicular traffic.
Special Conditions:
Any Contractor’s equipment that causes or generates electromagnetic disturbances or interference shall be removed from service until properly repaired. The Project Manager may also require repositioning or removal of the equipment from the Post.
Security Requirements:
The Contractor shall comply with all security regulations imposed by the base/post commander and/or agency occupying the space where work is to be performed. Any necessary security clearances shall be obtained prior to commencement of work.
The Contractor shall ensure that all parts of the facility where work is being performed are adequately protected against vandalism and theft.
See Section 7 – SECURITY REQUIREMENTS for additional information regarding security training requirements.
Work by Government:
The Government reserves the right to undertake performance by Government forces, for the same type or similar work as contracted herein, as the Government deems necessary or desirable, and to do so will not breach or otherwise violate this contract.
Regulations:
The Contractor shall comply with all applicable Federal, State, Local, DOD, National Guard Bureau, Army and Installation regulations pertaining to safety, traffic control, fire prevention, and Ravenna Environmental Requirements.
The Contractor, employees, and subcontractors are subject to, and shall abide by and comply with all relevant statutes, ordinances, laws and regulations of the United States (including Executive Orders of the President) and any State (or other public authority now or hereafter in force). The Contractor agrees to observe and comply with all applicable State and Federal requirements regarding social security, workman’s compensation, unemployment insurance and any other matters concerning employment applicable to the performance of this Contract or rules, regulations, directions and order not inconsistent herewith as may from time to time be issued by the Government. The unilateral act of any governmental body against any employee of the Contractor for the violation of a State or Federal law or regulation shall not excuse the Contractor from full compliance with the terms and conditions of this Contract.
The Contractor, employees, and subcontractors shall become familiar with and obey the regulations of the installation including fire, traffic, safety environmental and security regulations while on the military installation. Those driving motor vehicles shall observe and obey all speed limits posted throughout the installation. Personnel should not enter restricted areas unless required to do so and only upon prior approval. All Contractor and subcontractor employees shall carry personal identification on them at all times.
Contractor’s equipment shall be conspicuously marked for identification and parked or placed within approved areas only, out of the way of driveways, emergency access roads, and traffic. The Contractor shall ensure that the facility where work is being performed is adequately protected. The Contractor shall comply with all security regulations imposed by the base/post commander against vandalism and theft.
Safety Precautions and Programs:
OSHA Requirements:
The Contractor shall perform all work in the contract with due regard to the safety and health of the employees and of the public. The Contractor shall comply with 29 CFR 1926, Safety and Health Regulations for Construction, administered by the Federal Occupational Safety and Health Administration (OSHA) regarding the safety and protection of persons employed in construction and demolition work.
The Contractor shall develop and submit to the OHANG a project specific Health and Safety Plan covering all aspects of demolition and construction operations proposed by the Contractor for this project. Said project specific Health and Safety Plan shall address all aspects of 29 CFR 1926 as related to the Contractor’s proposed operations.
Provide adequate barricades, railings, overhead protection, safety nets, etc., to safeguard the public and workman from debris. Provide proper signs and written notification to all personnel and property owners in immediate and surrounding affected areas, notifying them of duration of construction operations.
Sidewalks and paths shall be protected, free of obstructions and maintained in a safe and satisfactory condition.
The Contractor shall provide, erect and maintain barricades, warning signs, flags, lights, as may be necessary to protect the work and safeguard the workman and the general public. All such protection shall comply with the requirements of the proper authorities having jurisdiction.
6 - Reference Standards This RFP references published standards, the titles of which can be found in the Unified Master Reference List (UMRL) on the Whole Building Design Guide at the Unified Facilities Guide Specification (UFGS) Website. The publications referenced form a part of this specification to the extent referenced.
The advisory provisions of all codes, requirements, and standards are mandatory; substitute words such as “must” or “required” for words such as “shall,” “should,” “may,” or “recommended,” wherever they appear. The results of these wording substitutions incorporate these code and standard statements as requirements. Reference to the “authority having jurisdiction” is interpreted to mean Contracting Authority or Contracting Authorities Representative. Comply with the required and advisory portions of the current edition of the standard at the time of contract solicitation.
The design and construction must be in accordance with established construction practices, and the latest revision/edition of the following referenced codes and standards. The term “Latest Revision/Edition” is defined as the version as of the project award date. References are available at www.wbdg.org/ndbm/. The advisory provisions of all codes and standards are mandatory, as though the word “must” had been substituted for “should” wherever it appears.

Provide work in compliance with the following design standards and codes, as a minimum. Government standards listed in this RFP take precedence over industry standards. The following list of codes and standards is not comprehensive and is augmented by other codes and standards referenced and cross-referenced in the RFP.

7 – SECURITY REQUIREMENTS
A. The CONTRACTOR shall ensure all personnel complete AT Level I training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility, or area. All contractor employees, including subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days after completion of training by all employees and subcontractor personnel. B. The CONTRACTOR shall ensure all personnel complete the Defense Security Service (DSS) computer-based course, OPSEC Awareness for Military Members, DoD Employees and Contractors (GS130.16) training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility, or area. All contractor employees, including subcontractor employees, requiring access to Army installations, facilities and controlled access areas shall complete OPSEC awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days after completion of the training by all employees and subcontractor personnel. 8 - Site VISIT A MANDATORY site visit will be held at Building 1068, 8451 SR 5 Ravenna, Ohio, 44266. The Date and Time of the site visit will be posted in the solicitation on sam.gov.

10 - PERFORMANCES
Submittal of a bid by a Contractor shall be accepted as prime facie evidence that the Contractor has examined the Specifications and the Job Site, as necessary and has satisfied themselves as to the nature and location of the work and all matters which can in any way affect the work or cost thereof, under the contract. Any failure of the Contractor to acquaint themselves with all available information, including physical survey of the proposed work, will not relieve them from successfully performing all work to be done for a complete and successful job.
The above brief outline of principle features of work in no way limits the responsibility of the contractor to perform all work and furnish all labor and materials, required by the specifications referred to herein. Any omission from the specification or misunderstanding thereof shall not relieve the Contractor from performing all the work required, and furnishing the materials required to completely fulfill the INTENT of the Contract.
The Contractor shall confine operations at the site to the area permitted by the Post Engineer. They shall not unreasonably encumber the site with materials or equipment and shall assume full responsibility for the protection and safekeeping of materials and equipment stored on site.
At the completion of each working day, the Contractor shall clean and clear away any excess supplies and debris from the site.
Upon completion and before final acceptance of the work, the Contractor is responsible for the removal of all rubbish generated by the project. All property shall be restored to safe normal operating conditions. The area shall be in a neat, clean, presentable condition satisfactory to the facility.
