About this file
This is a Solicitation Survey form (Solicitation No. W912WJ26QA173) that vendors must complete in its entirety to be deemed compliant and responsive to a federal contracting opportunity. Failure to provide all required information will result in the quote being deemed non-responsive.
The survey requires vendors to provide comprehensive company information including company name, SAM CAGE code, address, and point of contact details, along with designation of business type (corporation, partnership, sole proprietor, or other). All information submitted must pertain to the prime vendor submitting the quote unless specific circumstances allow subcontractor information. The form collects detailed information on business experience, including years of experience in relevant services, years of contracting experience as prime or subcontractor, years of federal government contracting experience, and any history of failed work completion. Vendors must identify key personnel who will perform work, specify minimum and maximum employee counts, confirm whether employees are on regular payroll, and disclose if additional hiring is necessary.
The survey requires documentation of specific certifications and licenses applicable to the work, including First Aid/CPR, Competent Person in Fall Protection, Rope Access, applicable trade licenses, Crane Operator/Qualified Riggers, and Boat Operator certifications. If certifications have not been completed, vendors must provide detailed plans for obtaining them within required timeframes. Vendors using subcontractors must provide complete subcontractor information (name, point of contact, location) with signed teaming arrangement documentation; stating information will be "provided upon request" results in non-responsive ratings. The form requires three past performance references completed within the past three years, with preference given to similar work in nature, size, and complexity, particularly government contracts. Each reference must include contract agency/company name, contract number, total value, points of contact with phone and email, contract dates, and detailed project scope information. All statements must be certified as complete and correct by an authorized official.
