C.1.1 Facilities Description
The USDOI - BLM operates a domestic, sanitary wastewater treatment system that uses chlorine disinfection and land application at the Shotgun Creek Recreation Site. The Recreation Site is 260 acres in size of which approximately 35 acres have been developed. The Shotgun Creek Recreation Site is a day use site that is open between May 1 and October 31. The facility has low winter flows to the treatment plant with intermittent winter land application.
The wastewater treatment system collects sewage from two centrally located rest rooms, a caretaker’s home, and an administration building that has a maintenance facility (See Figure 1).
BLM NORTHWEST OREGON DISTRICT Page 9 of 97
C.1.2 Wastewater Facilities Description
The treatment facilities consist of a collection system and a treatment system. The collection system has a single influent lift station which collects all sewage from the Creek and pumps it to the treatment system. The treatment system consists of a 72,190-gallon circular concrete aerated basin, a chlorine contact tank for disinfection, an effluent pump station and a land application site for recycled wastewater (See Figure 2). The facility has a SCADA system that is tied to the influent and effluent magnetic flow meters. The facility has a back-up power generator. Temperature, pH, and chlorine residual are tests performed at this facility.
The wastewater treatment is a plug flow aerated lagoon type system. Sewage enters the influent pump station where pH and flow are measured and then flows to the aerated basin. Flows from the aeration basin pass through a chlorine contact basin where chlorine is injected. After 60 minutes of detention in a metal contact chamber, chlorinated effluent is pumped to the land application site.
The disinfected effluent is then irrigated on the land application site in the forest. Irrigation in the summer months is accomplished through 21 micro-spray sprinkler heads distributed over the two-acre irrigation site which is divided into two zones. There are one, two, or three irrigation cycles of treated wastewater applied per day that are approximately 550 gallons each.
BLM NORTHWEST OREGON DISTRICT Page 10 of 97
C.1.3 Operations
The contractor shall be responsible for the operations of the wastewater treatment system in compliance with the existing wastewater permit 101895 and RWUP. A copy of the permit is attached as Attachment Task 1A;
copy of the RWUP is attached as Task 1B; a copy of the Shotgun Creek Emergency Response Plan is attached as Task 1C; and a copy of the Shotgun Creek Site and Disposal Area Layout is attached as Task 1D.
The contractor shall have the wastewater system supervised by one or more operators who are certified in a classification and grade level equal to or greater than Level 1. A "supervisor" is defined as the person exercising authority for establishing and executing the specific practice and procedures of operating the system in accordance with the policies of the permittee and requirements of the waste discharge permit. "Supervise" means responsible for the technical operation of a system, which may affect its performance, or the quality of the effluent produced. Supervisors are not required to be on-site at all times. The wastewater system may not be without supervision for more than 30 days. During this period, and at any time that the supervisor is not available to respond on-site (i.e., vacation, sick leave or off-call), the contractor shall make available another person who is certified in the proper classification and at grade level 1 or higher.
At all times the contractor shall maintain in good working order and properly operate as efficiently as possible all treatment or control facilities, or systems installed or used to comply with the terms and conditions of this PWS.
BLM NORTHWEST OREGON DISTRICT Page 11 of 97
At all times the contractor shall operate all facilities and/or systems as efficiently as possible in a manner that will prevent discharges, health hazards, and nuisance conditions.
No recycled water may be released by the contractor to another entity.
The contractor shall dispose of all screenings, grit, and sludge in a manner approved by ODEQ to prevent any pollutant from the materials from reaching waters of the state, creating a public health hazard, or causing a nuisance condition.
The contractor shall not be permitted to bypass untreated waste, except where unavoidable to prevent loss of life, personal injury, or severe property damage.
If the contractor is unable to comply with conditions of this contract and the permit because of surfacing sewage;
a breakdown of equipment, facilities or systems; an accident caused by human error or negligence; or any other cause such as an act of nature, the contractor shall:
● Immediately take action to stop, contain, and clean up the unauthorized discharges and correct the problem.
● Immediately notify the CO, and COR.
● Immediately notify the ODEQ Regional office so that an investigation can be made to evaluate the impact and the corrective actions taken, and to determine any additional action that shall be taken.
● Within five days of the time the contractor becomes aware of the circumstances, the contractor shall submit to ODEQ and the CO a detailed written report describing the breakdown, the actual quantity and quality of waste discharged, corrective action taken, steps taken to prevent a recurrence, and any other pertinent information.
The contractor shall notify ODEQ, CO, and COR within 24 hours if it is determined that the treated effluent is being used in a manner not in compliance with OAR 340-055. The contractor shall report the incident of noncompliance to the Oregon Emergency Response System (Telephone Number 1-800-452-0311). It shall also send a report to ODEQ no later than five days after the incident that details the circumstances, possible cause(s), and remedies. A copy of this report shall also be sent to the CO and COR on the same day the report is sent to ODEQ. Compliance with these requirements does not relieve the contractor from responsibility to maintain continuous compliance with the conditions of the permit or liability for failure to comply.
The following is a list of work items which shall define what work is included and how they shall be measured.
Any work item not mentioned shall be considered incidental.
C.1.3.1 Site Visit Inspection Form

date submitted; and submitted by shown as “Page X of X”; “Date Submitted” and “Submitted By” on each page.
The last page shall include a “Received by “and “Date” line for the COR signature. The contractor shall be responsible for keeping COR Signed and received date copies in a logbook available for the COR review within twenty-four (24) hours upon request. A draft copy shall be provided to the COR for their review within thirty
(30) days of the issuance of this contract. The COR shall have ten (10) days to review and comment on the proposed form. The contractor shall make any edits required and submit the updated form for approval within ten (10) days of receiving the COR comments.
