About this file

This is a Subcontractor Summary Worksheet instruction document for the City of St. Petersburg, Florida, related to a machine shop services procurement (IFB-26-215). The worksheet requires prime contractors to disclose all intended subcontractors at the time of bid submission and identifies that up to 80 percent of contract work may be subcontracted. The document specifies that subcontractors may be changed or substituted only under seven specific circumstances: failure or refusal to execute a written contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure or refusal to perform the subcontract, substantial underperformance as determined by the engineer, or substantial delay or disruption of work progress. Requests for subcontractor substitutions must be submitted in writing to the Procurement & Supply Management office, Contracts Compliance Manager, at Stephanie.Swinson@stpete.org. The worksheet requires bidders to identify each subcontractor's legal name, primary business address, project supervision contact information, tier classification (first or second tier only), specific trade classifications, and subcontract amount. Small Business Enterprises (SBEs) and Minority/Women Business Enterprises (MWBEs) used to meet participation requirements must be replaced with similarly certified firms if substituted, unless the prime contractor demonstrates that requirements are being met through alternative means.

The worksheet contains columns for tracking added and deleted subcontractors with corresponding reason codes and city approver signatures and dates. No restrictions on subcontracting percentage or multi-tier subcontractor usage apply to FDOT Locally Administered Program (LAP) or federally funded projects. The document emphasizes that completion and submission of this worksheet is mandatory with all bid submissions and serves as the official record for managing subcontractor changes throughout the contract term, which includes an initial three-year period with two optional one-year renewal periods.
