SF 1449 CONTINUATION - CONTRACT SCHEDULE

Managed Teleradiology Services

Solicitation No. 75H71226R00001

B.1 CONTRACT TYPE AND STRUCTURE

The Government contemplates award of one single-award, firm-fixed-price (FFP), indefinite-delivery/indefinite-quantity (IDIQ) contract for Area-wide Managed Teleradiology Services in support of participating Phoenix Area Indian Health Service (IHS) facilities. Services will be ordered in accordance with the terms of the resulting contract and applicable task orders or other authorized ordering procedures.

B.2 CONTRACT LINE ITEMS AND PRICING

The contract line items identified on the SF 1449 apply to services ordered under the contract. Offerors shall complete and submit Attachment 2 - Pricing Schedule. Attachment 2 is the required pricing submission and shall include firm-fixed, fully burdened unit prices for the Base Period and each option period, as applicable. The successful Offeror's awarded unit prices will be incorporated into the resulting contract.

Estimated quantities used for proposal evaluation are identified in Attachment 2 and the applicable Technical Exhibits. Prices entered in Blocks 23 or 24 of the SF 1449, if any, do not replace the requirement to complete Attachment 2.

B.3 ESTIMATED QUANTITIES

Quantities identified in Attachment 2 and the Technical Exhibits are estimates only and are provided for evaluation and planning purposes. Actual quantities may vary based on patient demand, staffing availability, operational requirements, recruitment and retention of Government radiologists, implementation of new imaging modalities, participation of additional facilities, and other mission requirements. Except for the minimum guarantee established below, the Government does not guarantee any quantity or dollar value associated with an individual CLIN, facility, modality, ordering period, or option period.

B.4 MINIMUM GUARANTEE

The minimum guarantee under the resulting IDIQ contract is $25,000.00. The minimum guarantee applies to the contract as a whole and not separately to any CLIN, facility, task order, ordering period, or option period. The Government's obligation with respect to the minimum guarantee is satisfied when the aggregate value of orders issued under the contract equals or exceeds $25,000.00. Except for this minimum guarantee, the Government does not guarantee any minimum quantity or dollar value of services.

B.5 MAXIMUM CONTRACT VALUE

The maximum aggregate value of all orders issued under the resulting IDIQ contract shall not exceed $25,000,000.00. The maximum contract value applies to the entire potential contract ordering period, including the Base Period and all exercised option periods. The maximum contract value is a limitation on aggregate ordering authority and does not constitute a representation or guarantee that the Government will order services up to that amount.
