About this file

This document outlines mandatory self-performance requirements for a contract with the Illinois Department of Human Services to install anti-slip pads on rooftops at 17019 County Farm Road, Rushville, Illinois. The contract term extends from the date of execution through June 30, 2027. The prime vendor must perform a minimum of 51% of total contract work on-site using its own organization, defined strictly as personnel employed and compensated directly by the prime vendor and equipment owned or leased by the prime vendor. Self-performance must be executed exclusively by permanent, full-time W-2 employees who have actively performed services for and received verifiable wages from the prime vendor for a minimum of 30 consecutive days prior to bid submission.

The document specifies that staff augmentation, 1099 independent contractors, and agency or temporary labor do not qualify toward self-performance requirements. Verification of the prime vendor's performance percentage may be conducted through the Illinois Department of Labor's Certified Payroll Report and facility visitor logs, with prime vendor employees required to carry identification supplied by the employer displaying the business name, employee name, and photo at all times. Non-compliance with the self-performance percentage at bid submission or post-award deviation without prior written approval from the State constitutes material breach of contract and grounds for immediate termination for cause.
