M A C S P O S T A T E M E N T O F W O R K

31 March 2022

Work Order: 9490415 Location: Building 1210 Project Title: Install New Shower Stall in Locker Room

1.0 General Information

1.1 Contractor shall provide all labor, materials, equipment, testing, transportation and services required to accomplish all work specified in accordance with requirements contained in the original contract specifications.

1.2 The contractor shall provide a TURN KEY proposal for all areas of the project, when completed and ready for final inspection shall meet all pertaining codes and governments’ statement of work

(SOW).

1.3 Only items incorporated into this document from the accepted proposal shall be considered part of this statement of work.

1.4 This project is intended to install new shower stall and vanity sink base in middle locker room of building 1210 in accordance with all applicable National, United States Air Force, State, and local building codes. All work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, Unified Facilities Criteria (UFC) state and federal codes, base standards, and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. Effective immediately: Due to safety concerns the use of manhole fixed ladders on Hanscom AFB is prohibited for the purpose of entry and egress is prohibited. An alternative means is authorized (i.e. appropriate portable ladder/personnel lowering device).

1.5 The Contractor shall provide requested material submittals with their proposals and provide all required as-build drawings, training, O&M manuals, and/or closeout documentation to the project manager prior to the final inspection as needed.

1.6 The contractor shall have three (3) days to complete any punch list items upon completion of the project and prior to 100% beneficial occupancy is granted, based upon final visual inspection/walk thru with the customer, PK contracting officer (CO), Project Manager, and contractor/sub-contractor(s).

1.7 The contractor shall clean up entire project area prior to final inspection and properly dispose of all construction materials/debris off base. The contractor shall provide completed waste diversion forms and weight tickets for construction debris disposal and/or recycled materials to the Project Manager upon completion of the project.

1.8 The contractor shall use qualified sub-contractors with the necessary work experience to provide a quality finished product.

1.9 The contractor and sub-contractors must visit and inspect jobsite prior to bidding the project.

After or upon contractor inspection of jobsite, contractor may notify the Contracting Officer of any questions or deficiencies that might require modifications to the government statement of work (SOW), prior to contract award.

1.10 The contractor shall conform to all safety requirements contained in all applicable standards, for activities related to the accomplishment of the work performed and provide dust protection as reasonable, to protect government assets.

1.11 The contractor shall be responsible for safeguarding all government property. At the close of each work period, government facilities, property, and materials shall be secured.

1.12 The contractor shall provide protection to government property, to prevent damage during the period of time the property is under the control or in possession of the contractor.

1.13 The contractor shall record and report promptly (within one hour) to the contracting officer or Project Manager, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

1.14 In the event of an accident/mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

1.15 If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

1.16 The contractor shall notify the Project Manager of any environmental spills, prior to reporting the incident to the Hanscom AFB Fire Dept. at 911. If the contractor spills or releases any substance as contained in 40 CFR 302 into the environment, the liability for the spill or release of such substances rests solely with the contractor and its agent to remediate the area as required, at no additional cost to the government.

1.17 The Prime and sub-contractors shall warranty all materials and installation for a period of one year upon completion of the project.

1.18 The contractor shall implement and maintain a three phase inspection and quality control system through the life of the design, construction and closeout of the delivery order.

1.19 The contractor shall clean-up work in the interior/exterior of building area at end of each day.

1.20 The contractor shall not use government furnished roll-off containers and/or dumpsters.

Contractor(s) shall provide their own containers for construction debris.

2.0 General Scope of Work

Submittals are not required after award.

2.1 The contractor shall install new shower stall and sink. The contractor is responsible for field verifying all quantities and measurements.

2.2 The contractor shall remove the following from room. (Approximate room size 11’x11’)

(150) Square Feet of Carpet

(150) Square Feet of Ceiling and Grid

(400) Square Feet of old Drywall

2.3 The contractor shall demo all plugs, switches, overhead lights, drywall, return vent, carpet, ceiling tiles and ceiling grid, all wiring to be demo’d back to nearest junction box in ceiling

2.4 The contractor shall disconnect heat sensor in old ceiling, (save for re-install)

2.5 The contractor shall remove old wall heater

2.6 The contractor shall move all pipes into wall and insulate as needed

2.7 The contractor shall install shut off valves for new baseboard heat

2.8 The contractor shall install new slim-line baseboard heater (See Picture Bathroom Window 2)

2.9 The contractor shall frame in, and green board in other water pipe on exterior wall and put in access panel for shut-off valves and cover wall with tile. (See Picture Bathroom 2)

2.10 The contractor shall add a new exhaust vent in new ceiling to be vented outside thru roof

2.11 New exhaust fan Medium profile bathroom fan, 110CFM, 6” inch duct diameter, 120V, 60Hz, 600 RPM or similar. (See Electrical Locations).

2.12 All roof penetrations need to be flashed IAW manufacturer’s recommendations

2.13 The contractor shall remove existing window and dispose of

2.14 The contractor shall frame in existing opening for two (2) small privacy windows. Patching exterior to match existing finish (EIFS) (See Picture Bathroom Window)

2.15 Two (2) Small Privacy Windows should be approximately Twenty Four (24’’) inches Wide X Twelve (12”) inches High, Double pane frosted, Bronze tone extruded aluminum (Store Front)

2.16 The contractor shall remove extra wood door leading to men’s locker room and dispose of.

(See Picture Shower Stall and Vanity)

2.17 The contractor shall frame in door that was removed and insulate with R-15 insulation and install green board

2.18 Locker room side to receive a level (4) four finish, with one (1) coat of latex primer and two

(2) coats of latex finish paint to match existing. Install cove base to match existing

2.19 The contractor shall trench floor for new drain and vent for shower and sink

2.20 Main drain pipe is approximately eleven (11) feet away in the bottom of the opposite wet wall where female bathroom is. (See Picture Electrical Locations).

2.21 New hot and cold water lines to be installed as needed from mechanical room approximately Fifty (50) feet away (See Picture Mech. Room Location) all new water feed lines in ceiling and walls need to be insulated

2.22 The contractor will install new shower and sink drain pipe, tie in to existing drain, pin existing cement as needed, cement trench level with existing floor

2.23 The contractor shall install new shut-off valves under sink.

2.24 The contractor shall insulate all new bathroom walls with R-15 insulation, install 5/8” green board, finish to receive wall tile

2.25 The contractor shall install a new drop ceiling and new ceiling grid. Use Spectra Tile Finale White Moisture Resistant Nonporous Ceiling tiles or Equal. Reinstall saved heat sensor. All new lights wiring and exhaust vent to be wired to junction boxes above ceiling.

2.26 The contractor shall install Two (2) new ceiling lights. (See Picture Electrical Locations).

2.27 Coronet TDJA LED 2 X 4 Lights, Two (2) LGT-1 Standard, 4000K, UNV. T-9/16” Tee Grid, TZ-9/16” Tech Zone, F-Flange, SG-Slot Grid/Interlude, DB-Standard 10% Dimming (0-10V).

2.28 The contractor shall install Three (3) white GFCI (Ground Fault Circuit Interrupter) and One

(1) white dual light/fan switch, with white covers. (See Picture Electrical Locations).

2.29 The contractor shall install new wiring to new GFCI plugs, and switches from junction boxes in ceiling

2.30 The contractor shall tile all four (4) walls from floor to ceiling using Dal Tile semi-gloss/matte color group arctic white 0190, semi-gloss (CT-2). Wall grout to be 00-white (Blanco).

(See Picture Shower Stall and Vanity) or equal.

2.31 Floor tile will be Dal Tile Suede Gray Speckle, D208, (CT-1), Floor grout 27-silver (Plata) or equal.

2.32 The contractor shall install Wolf Classic Impact Series Somerset White paint vanity sink base, Two (2) Butt Doors, One false panel, VSB36. (See Picture Shower Stall and Vanity) or equal.

Thirty Six (36”) inch White Vanity Sink Base or equal. With mirror mounted to wall above sink 36” x 36” Thirty Six (36”) inch Solid Surface Corian Counter Top with back splash, Sparkling White) or equal

2.33 Kohler, Pennington, Drop-in bathroom sink, K-2196-4, Color Code O (White) or equal.

2.34 Chicago Faucets, Part 895-317E29XKABCP, Deck-Mounted, Manual Sink Faucet, with four (4”) inch centers, 3-1/2” Rigid Goose Neck, Pressure compensating laminar flow non-aerating outlet four (4”) inch wrist blades, polished chrome or equal.

2.35 The contractor shall install a Forty Eight (48”) inch Wide X Eighty (80”) inch high Shower Wall Kit. (See Picture Shower Stall and Vanity) or equal.

Syn-Mar Shower Wall Kit, Platinum White or equal.

Syn-Mar Shower Base Pan, Platinum White or equal.

2.36 The contractor shall install Signature Series Shower Arm S-Type, SKU-908108, with 12” inch vertical rise and 17-1/2” extension into the shower with downward connection threads or equal

2.37 The contractor shall install a Symmons Rain shower head 4-166, Polished Chrome or equal

2.38 The contractor shall install a Bobrick Classic Series, Extra Heavy Duty shower curtain rod. B- 6047, Forty Eight (48”) inch Wide X Seventy Two (72”) High or equal.

2.39 The contractor shall use Bobrick Stainless Steel shower curtain hooks, 204-1, and Bobrick vinyl shower curtain. 204-3 or equal

2.40 The contractor shall make sure all new bathroom faucets and drains are working properly.

3.0 Warranty:

3.1 The Contractor shall also provide a 1 year contractor installation warranty.

4.0 Project Requirements:

a. The Period of Performance (PoP) for this project shall be 120 days from receipt of delivery order, unless directed otherwise by the Contracting Officer. Time adjustments will be made due to weather delays.

b. Contractor shall perform work Monday thru Friday from 0600-1700, work outside of these hours must be approved and coordinated with the Project Manager and User.

Contractor will be required to be escorted by government representative at all times.

c. Contractor shall coordinate with the project manager all work prior to starting the contract and during the performance with the project manager. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.

d. Contractor shall ensure that the job site is clean before leaving at the end of each workday.

e. (if applicable) The contractor will coordinate with the contracting officer and the project manager to obtain base access badges for contractor personnel. The contractor is responsible to immediately return all access badges for personnel who no longer work for the contractor. The contractor is responsible to immediately return all access badges after completion of the contract.

f. Contractor shall provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates. Provide resolutions of any technical problems that arise during the construction.

g. Quality Assurance: The government will evaluate the contractor’s performance for each job.

h. Government Remedies: The contract officer shall follow the requirements of FAR 52.212- 4, contract terms for Commercial Items, for contractor’s failure to correct non-conforming services.

i. Submittals: The contractor shall provide submittals on all materials being installed for approval prior to starting the project.

j. Any modifications and addition on a facility that contains Lightning Protections System (LPS) This includes paint projects, roofing projects, projects requiring roof penetrations, installation of new HVAC or other metallic equipment, antenna installation, or other work.

Additions and modifications to the facility envelope require re-evaluation of the LPS for the completed facility as a whole. The LPS required by the addition or modification shall not be a simple addition to the LPS. The LPS must be considered a single entity and must comply with requirements of a single LPS upon completion. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order. Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final LPS in the initial design. Projects of this type shall ensure the final LPS as a whole is compliant with AFI 32-1065 and NFPA 780, in that priority order. The same third-party inspector requirements shall apply.

k. The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.
