1. HEALTH AND SAFETY SCOPE OF SERVICES
1.1. Safety and Health Requirements
The Contractor shall review all available site information and develop the necessary documents containing health and safety criteria, procedures, and work practices sufficient to protect on-site personnel, the environment, and potential off-site receptors from hazards particular to each site. The Contractor shall utilize the services of qualified personnel to oversee the development and implementation of required safety and health documents, site designs/work plans, and field oversight to ensure compliance with all applicable laws and regulations.
A Safety and Health Manager (SHM) is required for projects/task orders issued under this contract.
The SHM, dependent upon the contaminant-related hazards on the project, shall be a Certified Industrial Hygienist, Certified Safety Professional, or Certified Health Physicist (CHP) and have a minimum of three years of experience managing safety and occupational health at hazardous waste site cleanup (or radioactive waste sites, if necessary) operations. If the project involves radioactive material, then the Contractor shall assign a CHP as the SHM or in support of the SHM. If the project involves intrusive activities, the Site Safety and Health Officer or SSHO must have the OSHA 30-hour training, as well as the OSHA training identified in 1910.120 per EM 385-1-1. All competent-person training shall be completed per EM 385-1-1 and OSHA, as applicable.
1.2. Regulatory Requirements
All site investigation, construction, monitoring, operations and maintenance activities and health and safety documents required by this scope of services shall comply with pertinent sections of the following regulations and reflect the following guidance publications:
• Federal Acquisition Regulation, F.A.R. Clause 52.236-13: Accident Prevention.
• U.S. Army Corps of Engineers (USACE), Safety and Health Requirements Manual, EM 385-
1-1 (latest revision) - http://www.usace.army.mil/Safety-and-Occupational-Health/
• Nuclear Regulatory Commission Standards, 10 CFR 20 – Standards for Protection Against
Radiation.
• Occupational Safety and Health Administration (OSHA) Construction Industry Standards, 29

1.3. Safety and Health Program Documents
OSHA requires all employers performing on-site activities at hazardous waste sites to follow OSHA Standard 29 CFR 1910.120and 29 CFR 1926.65. The Contractor shall develop a Safety and Health Program which meets these requirements, to include the appropriate documents as described below.

1.3.1. Accident Prevention Plan
Before initiation of work at the job site, an Accident Prevention Plan (APP) shall be reviewed and found acceptable by the Contracting Officer. The APP shall contain appropriate appendices (for example, a Site Safety and Health Plan (SSHP) for hazardous waste site cleanup operations, a Lead Compliance Plan when working with lead, an Asbestos Hazard Abatement Plan when working with asbestos, and/or a hazardous energy control program, if required).
• APPs shall be developed and submitted by the Contractor in the format provided in Appendix A of the EM 385 1-1. The Contractor shall address each of the elements/sub-elements in the outline contained in Appendix A in the order that they are provided in the manual. If an item is not applicable because of the nature of the work to be performed, the Contractor shall state this exception and provide a justification.
• The APP shall be developed by qualified personnel and then signed in accordance with EM 385-1-1. The Contractor shall be responsible for documenting the qualified person’s credentials.
• The Contractor’s APP shall be job specific and shall include work to be performed by subcontractors. In addition, the APP should state measures to be taken by the Contractor to control hazards associated with materials, services, or equipment provided by suppliers.
• The APP shall provide for frequent safety inspections/audits, conducted by a competent person, of the work sites, material, and equipment to ensure compliance with the plan and EM 385-1-1. These inspections/audits shall be documented in writing and available upon request to the Contracting Officer. They shall include the name of the inspector, date, and all findings.
1.3.2. Site Safety and Health Plan, Appendix to the Accident Prevention Plan
The APP/SSHP shall cover each SSHP element in Section 33.B of EM 385-1-1. SSHP appendix elements that overlap with APP elements are not to be duplicated in the SSHP. The SSHP required by 29 CFR 1910.120(b)(4)/29 CFR 1926.65(b)(4), EM-385-1-1, and ER-385-1-92 shall be prepared and submitted. On-site activities shall not commence until the SSHP has been reviewed and accepted. The SSHP shall describe the site-specific safety and health procedures, practices, and equipment to be implemented and utilized to protect affected personnel from the potential hazards associated with the site-specific tasks to be performed. The level of detail provided in the SSHP shall be tailored to the type of work, complexity of operations to be accomplished, and hazards anticipated.
• The APP/SSHP shall be developed under the direct supervision of a qualified SHM.
• On-site implementation and enforcement of the APP/SSHP shall be managed by a qualified
SSHO.
• Changes and modifications to the SSHP are permitted and shall be made in writing with the knowledge and concurrence of the SHM and accepted by the Contracting Officer.
• If projects involve radioactive or mixed waste remediation, the Contractor shall also follow
EM 385-1-80.
1.4. Document Revisions, Addenda, and Field Modifications
Review comments issued prior to SSHP approval shall be incorporated by revision and reissuing affected pages. Minor changes affecting only a few pages may be made by addenda sheets and resubmitted. Once on-site, unanticipated field conditions encountered, which were not addressed in the approved SSHP, shall be immediately reported to the USACE point of contact. Field activities in such areas shall be halted until the SSHP has been modified to reflect changed conditions and reviewed and accepted by USACE.
