About this file
This is a Quality Assurance Surveillance Plan (QASP) document developed in accordance with Federal Acquisition Regulation (FAR) 46.401 for monitoring contractor performance on portable and vault toilet rental and servicing at the Golden Gate National Recreation Area (GGNRA), South District. The QASP establishes systematic methods for evaluating the contractor's compliance with a Performance Work Statement (PWS), identifying performance objectives, surveillance methods, performance thresholds, and the responsibilities of Government personnel. The plan emphasizes that the contractor—not the Government—bears responsibility for day-to-day management, quality control, and performance of all contract services. Government surveillance will focus on verifying that portable toilet rental, servicing, vault toilet pumping, waste disposal, optional services, and associated documentation are performed in accordance with performance standards established in the PWS.
The QASP defines roles for the Contracting Officer (CO), Contracting Officer's Representative (COR), and contractor, with the COR serving as the primary point of contact for day-to-day contract administration. Performance assessment includes nine key objectives: portable toilet rental and placement (100% compliance with approved schedules), portable toilet servicing (no more than three validated deficiencies per month), cleanliness and sanitation (no more than three validated deficiencies per month), toilet supplies (no more than three validated deficiencies per month), unit condition (no more than three validated deficiencies per month), graffiti and damage management, vault toilet pumping (100% completion within required response times), waste disposal (zero unauthorized incidents), and environmental protection (zero unauthorized spills). The plan authorizes surveillance methods including periodic and random inspections, service log reviews, complaint investigations, and documentation review. Critical deficiencies affecting public health, safety, accessibility, or environmental protection must be corrected immediately; major deficiencies must be corrected by the next scheduled service; and minor deficiencies during the next scheduled service. The document includes an appendix with a COR Inspection Checklist template for documenting performance assessments across portable toilets, vault toilets, environmental compliance, and documentation requirements.

1. OBJECTIVE
This Quality Assurance Surveillance Plan (QASP) is developed in accordance with Federal Acquisition Regulation (FAR) 46.401 to provide a systematic method for evaluating the Contractor's performance under the Performance Work Statement (PWS) for Portable and Vault Toilet Rental and Servicing at the Golden Gate National Recreation Area (GGNRA), South District.
The purpose of this QASP is to establish the Government's quality assurance surveillance methods for monitoring Contractor performance and verifying that services are performed in accordance with the requirements of the PWS. The QASP identifies the Government's performance objectives, surveillance methods, performance thresholds, and the responsibilities of the Contracting Officer (CO) and Contracting Officer's Representative (COR) for assessing contract performance throughout the period of performance.
This QASP is based on the principle that the Contractor—not the Government—is responsible for the day-to-day management, supervision, quality control, and performance of all contract services. The Contractor shall establish and maintain an effective Quality Control (QC) program to ensure all work complies with the requirements of the contract. The Government will evaluate Contractor performance through surveillance methods described in this QASP and will not duplicate the Contractor's quality control responsibilities.
Government surveillance will focus on verifying that portable toilet rental, servicing, vault toilet pumping, waste disposal, optional services, and associated documentation are performed in accordance with the performance standards established in the PWS.
Surveillance methods may include periodic inspections, random spot inspections, review of service documentation, validation of customer complaints, and other methods necessary to confirm contract compliance.
The Government intends to maintain a cooperative working relationship with the Contractor throughout the contract period to promote continuous improvement, resolve performance issues promptly, and ensure the delivery of clean, sanitary, safe, and accessible restroom facilities that support visitor health, resource protection, and the mission of the National Park Service.
2. MULTI-FUNCTIONAL TEAM (MFT) ROLES AND RESPONSIBILITIES
The Government's Multi-Functional Team (MFT) is responsible for contract administration, performance monitoring, and communication with the Contractor throughout the period of performance. The MFT will monitor Contractor performance in accordance with the
Performance Work Statement (PWS), this Quality Assurance Surveillance Plan (QASP), and applicable Federal Acquisition Regulation (FAR) requirements.
The Government's quality assurance program is intended to verify that the Contractor is meeting the contract requirements. It is not intended to direct or supervise the Contractor's day-to-day operations or replace the Contractor's responsibility for quality control.
2.1 Contracting Officer (CO)
The Contracting Officer (CO) is the only Government official authorized to enter into, administer, modify, interpret, or terminate this contract.
The CO is responsible for:
• Providing overall contract administration and ensuring compliance with applicable procurement regulations.
• Reviewing Contractor performance and ensuring compliance with the terms and conditions of the contract.
• Resolving contractual issues that cannot be resolved by the COR.
• Reviewing documented performance deficiencies and determining appropriate contractual remedies when necessary.
• Issuing contract modifications, exercising option periods, and approving changes to the contract.
• Reviewing Contractor performance information for annual Contractor Performance Assessment Reporting System (CPARS) evaluations.
• Providing final decisions regarding interpretation of contract requirements.
Only the CO has the authority to direct the Contractor to perform work outside the scope of the contract or to modify any contract requirement.
2.2 Contracting Officer's Representative (COR)
The Contracting Officer's Representative (COR) is designated in writing by the Contracting Officer and is responsible for monitoring Contractor performance and documenting compliance with the requirements of the PWS.
The COR is the Government's primary point of contact for day-to-day contract administration and quality assurance activities.
The COR's responsibilities include:
• Monitoring Contractor performance through inspections, service log reviews, and other surveillance methods identified in this QASP.
• Verifying that portable toilet rental, servicing, vault toilet pumping, optional services, and documentation are performed in accordance with the PWS.
• Conducting periodic and random inspections of portable toilets, ADA-compliant units, and vault toilet locations.
• Reviewing Contractor service documentation, service log stickers, and supporting records.
• Documenting performance deficiencies and notifying the Contractor of unacceptable performance.
• Verifying that identified deficiencies have been corrected within the required timeframe.
• Coordinating with park staff regarding visitor complaints, operational issues, and service scheduling.
• Maintaining inspection records and surveillance documentation in the official contract file.
• Recommending acceptance or rejection of services to the Contracting Officer based on Contractor performance.
• Assisting the Contracting Officer in preparing annual Contractor Performance Assessment Reporting System (CPARS) evaluations.
The COR shall not:
• Direct the Contractor to perform work outside the scope of the contract.
• Modify the contract.
• Change the contract price or period of performance.
• Authorize additional work not specifically provided for in the contract.
• Waive contract requirements.
Any request that may affect the scope, cost, or terms of the contract shall be referred to the Contracting Officer for resolution.
2.3 Contractor
The Contractor is solely responsible for managing and performing all services required by the contract and for maintaining an effective Quality Control (QC) program.
The Contractor shall:
• Perform all services in accordance with the PWS and approved service schedules.
• Provide qualified personnel, equipment, vehicles, materials, and supplies necessary to meet contract requirements.
• Conduct routine quality control inspections to verify compliance with the contract.
• Correct deficiencies identified through Contractor quality control or Government surveillance.
• Maintain accurate service documentation and provide records to the COR upon request.
• Notify the COR of conditions affecting contract performance, including emergencies, access issues, equipment failures, or environmental incidents.
• Cooperate with the Government during performance assessments and inspections.
The Contractor remains fully responsible for the quality and timeliness of all services performed under this contract, regardless of the level of Government surveillance.
3.0 PERFORMANCE ASSESSMENT
The contract service requirements are summarized in the Performance Assessment Table below. The table identifies the Performance Work Statement (PWS) paragraph associated with each service, the minimum acceptable performance threshold, and the Government's method of surveillance.
The Contractor's Quality Control (QC) Program and the Government's Quality Assurance Surveillance Plan (QASP) are intended to complement one another. The Contractor is responsible for performing all services in accordance with the contract requirements. The Government will monitor Contractor performance using the surveillance methods identified in this section to verify compliance with the PWS.
Inspection and surveillance activities will be based on performance outcomes rather than continuous oversight of the Contractor's daily operations. The Government reserves the right to increase or decrease surveillance based on the Contractor's demonstrated level of performance.

Repeated failure to meet the performance thresholds identified in this QASP may result in one or more of the following actions, as appropriate:
• Re-performance of services at no additional cost to the Government.
• Written notice of performance deficiencies.
• Increased Government surveillance.
• Documentation in the Contractor Performance Assessment Reporting System
(CPARS).
• Other contractual remedies available under the contract and applicable Federal Acquisition Regulation (FAR) provisions.
4.0 PERFORMANCE MANAGEMENT
The Government and the Contractor shall work cooperatively throughout the period of performance to ensure contract requirements are consistently achieved and to promote continuous improvement in the delivery of portable and vault toilet services.
Performance management consists of the Contractor's Quality Control (QC) Program, Government Quality Assurance surveillance, documentation of contract performance, and periodic evaluation of Contractor performance. The Government's surveillance activities are intended to verify compliance with the contract requirements and are not intended to replace or duplicate the Contractor's responsibility for quality control.
4.1 Contractor Quality Control
The Contractor is solely responsible for the quality of all services performed under this contract and shall establish, implement, and maintain an effective Quality Control (QC) Program throughout the period of performance.
The Contractor's QC Program shall ensure that contract requirements are consistently met before Government inspections occur.
At a minimum, the Contractor shall:
• Perform routine quality control inspections of all portable toilets and vault toilets serviced under the contract.
• Verify that all scheduled services have been completed in accordance with the approved Service Schedule.
• Ensure portable toilets are clean, sanitary, deodorized, fully stocked, and in proper working condition following each service.
• Verify that vault toilet pumping and cleaning have been completed in accordance with the PWS.
• Maintain current service log stickers on each portable toilet.
• Promptly identify and correct deficiencies discovered during Contractor inspections.
• Maintain documentation of inspections and corrective actions.
• Notify the COR of conditions that may affect contract performance, including damaged units, vandalism, severe weather, access limitations, or environmental incidents.
The Contractor shall continuously evaluate the effectiveness of its Quality Control Program and implement improvements necessary to maintain compliance with contract requirements.
The Government's review of the Contractor's Quality Control Program does not relieve the Contractor of responsibility for meeting all requirements of the contract.
4.2 Government Surveillance
The Government will monitor Contractor performance using the surveillance methods identified in this QASP to verify compliance with the Performance Work Statement (PWS).
Government surveillance may include:
• Scheduled inspections of portable toilet locations.
• Random, unannounced inspections.
• Review of Contractor service logs and service log stickers.
• Observation of Contractor servicing activities.
• Verification of scheduled service frequencies.
• Inspection of vault toilet pumping services.
• Review of visitor and employee complaints.
• Review of Contractor corrective actions.
• Verification of environmental compliance and spill response.
The frequency of Government surveillance may vary throughout the contract period based on the Contractor's demonstrated level of performance.
When Contractor performance consistently meets or exceeds contract requirements, surveillance may be reduced. Conversely, surveillance may be increased when recurring deficiencies, validated complaints, or other performance concerns are identified.
Government surveillance is intended to verify Contractor performance and shall not be interpreted as Government direction or supervision of Contractor employees.
4.3 Performance Meetings
The COR and Contractor may conduct periodic performance meetings throughout the contract period to discuss overall contract performance.
Performance meetings may include discussion of:
• Inspection results.
• Customer complaints.
• Performance trends.
• Recurring deficiencies.
• Corrective actions.
