About this file

This is a Supplier Diversity Program (SDP) Spending Report form from the Commonwealth of Massachusetts, designed for prime contractors to document their quarterly expenditures with certified diverse business partners. The form requires contractors to report all spending with SDP partners within 45 days of the end of each quarter, covering four quarterly periods: Quarter 1 (July 1–September 30), Quarter 2 (October 1–December 31), Quarter 3 (January 1–March 31), and Quarter 4 (April 1–June 30). Contractors must provide their company name, contact information, annual SDP spending commitment percentage, and quarterly contract sales figures. The form structure requires contractors to identify each SDP partner by name, relationship type (subcontracting or ancillary products and services), and certification category, with separate line items for each partner's quarterly spending amounts.

SDP partners must hold at least one certification from recognized categories including MBE (Minority Business Enterprise), WBE (Woman Business Enterprise), M/WBE, SDVOBE (Service-Disabled Veteran-Owned Business Enterprise), VBE (Veteran-Owned Business Enterprise), DOBE (Disability-Owned Business Enterprise), LGBTBE, or nonprofit variants (M/NPO, W/NPO, M/W/NPO). Contractors certified as diverse businesses themselves cannot list their own organization as an SDP partner. The form includes a year-to-date compliance check for subcontracting spending and requires contractors to explain any noncompliance with their annual spending commitments and describe corrective actions. The submission requires certification under pains and penalties of perjury, with the contractor's authorized representative providing name, title, phone, email, and date. The Supplier Diversity Office reserves the right to contact SDP partners to verify reported payments.
