SECTION C: INL SOW-SPECIFICS PER PROJECT

A004

HAITI MODULAR UNITS DESIGN AND BUILD

PER LOCATION (CLINs)

1. CLIN 001-Canadian Embassy-Delmas 75, Port-au Prince, Haiti

The contractor shall dismantle, remove, pack ready to go, and transport nineteen (19) Government Furnished Material (GFM) located at the Canadian Embassy– Port au Prince.

This project includes, but it is not limited to site work, removal of existing fence, disconnection and removal of utility lines (water, sewer, electric), dismantlement of aluminum stairways and transportation of the stairways to a HNP site in Port au Prince. Decommissioning and removal of Modular Housing Units from their foundation, loading of the Modular Housing Units on to flatbed trucks, and the transportation of the Modular housing units to City Haul Port au Prince, Les Caye CG Base, Sea Port au Prince, Terrier Rouge Customs Terrier Rouge, and DCPJ Port au Prince. For the restoration of site, see J-2-05 GFM Inventory from Canadian Embassy and J- 2-04 INL Project Conceptual Details/Drawings. PRNCE Mission to provide their drawings c/w specification notes to the U.S. Embassy for inclusion in the solicitation process. For each modular unit being removed, pull back all existing cabling from the modular to the telecom closet in coordination with the Embassy of Canada Foreign Service IT Professional (FSITP). All cables to be removed with be properly identified and removed from the terminal block within the telecom closet by the FSITP.

Additional requirements include:

a) Conduct and document a site survey and u/g services locate in areas of construction, areas of materials staging and storage and vehicular travel areas/paths to confirm exact location and type of u/g service affected by Contractor’s work in order to protect these areas from construction activities and impacts.

b) When performing at this facility, through the INL COR, the contractor shall provide performance schedule, which sequences work to minimize disruption at the job-site.

c) All disconnected utilities shall be capped and/or adjusted, re-instated and tested/commissioned to assure the complete functionality of the utilities and services for the MHUs not being dismantled and transported off-site.

d) Main utilities supporting the compound can be temporarily turned off while decommissioning is in progress for periods of time during the day but reestablished at the end of the work day.

e) For each MHU being removed, pull back all existing cabling from the MHU to the telecom closet in coordination with the Embassy of Canada Foreign Service IT Professional (FSITP). All cables to be removed with be properly identified and removed from the terminal block within the telecom closet by the FSI

f) Restoration of site (INL Haiti explains what does this exactly mean, it’s confusing).

Time is of the essence in the execution of this project and special attention will be placed on the safety practices of the Contractor. The Canadian Embassy, Haiti is an active site. A safety plan must be submitted for the entire project prior to the start of any site work and construction.

Contractor shall closely follow instructions provided in the general SOW addressing post construction/performance service activities.

1.1 General Requirement

All components of the modular units are currently located at the Canadian Embassy, Port au Prince, Haiti. Contractor shall:

1.1.1 Identify containers at Canadian Embassy, a jointly process will be coordinated to mark and inventory parts

1.1.2 Decommission identified Units.

1.1.3 Dismantle identified units

1.1.4 Pack ready –to-go and load specified units on separate conveyance and transport to:

a) Les Cayes CG Station

b) Terrier Rouge Customs

c) Sea Port BLTS

d) DCPJ

1.1.5 Restore site

1.2 Special instructions

Damage to persons or property

The Contractor must obtain All Risks Property insurance while the Government of Canada Property is under its care, custody or control. The contractor shall handle FGM as described in the general SOW throughout the duration of the Contract. The estimated dollar value of the GFM is estimated to be $5 million. The Government's Property must be insured on Replacement Cost basis.

1.2.1 Administration of Claims: The Contractor must notify the U.S. Government and

Canada promptly about any losses or damages to Government of Canada Property and monitor, investigate and document losses of or damage to ensure that claims are properly made and paid.

1.2.2 The All Risks Property insurance policy must include the following:

a. Notice of Cancellation: The Insurer will endeavor to provide the Canadian

Embassy in Port-au-Prince at least thirty (30) days written notice of policy cancellation.

b. Loss Payee: Canada as its interest may appear or as it may direct.

c. Waiver of Subrogation Rights: Contractor's Insurer to waive all rights of subrogation against Canada as represented by the Department of Foreign Affairs, Trade and Development of Canada for any and all loss of or damage to the property however caused.

1.3 Operations and storages Areas – Canadian Embassy

1.3.1 Confinement to Authorized Areas - The Contractor shall confine all operations (including storage of materials) on Government of Canada premises to areas authorized or approved by the COR. The Contractor acknowledges and accepts at its own risk that Canada, on the basis of sovereign power, may restrict at any time access to the grounds of the Canadian embassy in Port-au-Prince.

1.3.2 Vehicular Access - The Contractor shall and in accordance with any regulations prescribed by the Government of Canada, use only established site entrances and roadways

1.3.3 The contractor shall implement this and all the requirements described in the general SOW while performing services under this contract.

1.4 Accident Prevention

1.4.1 General - The Contractor shall provide and maintain work environments and procedures which shall (1) safeguard the public, Government of Canada personnel, U.S. Government personnel, visitors to the grounds or building of the Embassy of Canada in Haiti, property, materials, supplies, and equipment exposed to Contractor operations and activities; (2) avoid interruptions of Government of Canada operations and delays in project completion dates; and (3) control costs in the performance of this Task Order modification requirement. For these purposes, the Contractor shall -

1.4.2 Provide appropriate safety barricades, signs and signal lights;

1.4.3 Comply with the standards issued by any local government authority having jurisdiction over occupational health and safety issues; and

1.4.4 Ensure that any additional measures the Contracting Officer determines to be reasonably necessary for this purpose are taken.

1.5 Approval of sub-contractors

In addition to the requirements described in the general SOW, the contractor, while performing services at the Canadian Embassy, shall proceed as follows:

1.5.1 Review and approval: The Government reserves the right to review proposed subcontractors for a period of five (5) calendar days before providing notice of approval or rejection of any or all subcontractors. Contractor shall obtain Consent to Subcontract prior to initiating work.

1.5.2 Exclusion of persons: The Contractor acknowledges and accepts that Canada may at any time prohibit access to the grounds of the Canadian Embassy in Port-au- Prince to any number of persons. In all cases, the Contractor shall have no recourse or remedy against Canada or the U.S. Government as a result of this prohibition.

1.5.3 Rejection of subcontractors: The Government reserves the right to reject any or all subcontractors proposed if their participation in the project, as determined by the Contracting Officer, may cause damage to the national security interests of the United States or of Canada. The Contractor agrees to promptly replace any subcontractor rejected by the U.S. Government under this clause.

2. CLIN 002: Les Cayes Coast Guard Station (CG)-Les Cayes, Haiti

The Les Cayes CG station has an existing open area in front of the CG Station between the station and the boat docking area. This project includes, but it is not limited to site work and installation of pre-fabricated/modified office and housing containers, connection to existing utilities (electric, water, sewage), size, provide/install a backup electrical generator with Automatic Transfer Switch (ATS) and using existing water supply, provide/install a water system. Site work will include clearing, grading and compacting existing area prior to footing installation for the nine (9) pre-assembled GFM (Canadian Embassy), one (1) modular ablution unit and one (1) twenty foot shipping container. See J-2-06 GFM inventory-US Embassy and J- 2-04 INL Project Conceptual Details/Drawings.

The Contractor shall be solely responsible for providing layout design concepts, procurement and installation of modified office/housing containers, construction services, utility and sewage connections, contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this task order modification within the established schedules, and in accordance with all of the main contract requirements. Time is of the essence in the execution of this project and special attention will be placed on the safety practices of the Contractor.

The CG Base at Les Cayes is an active Coast Guard station. A safety plan must be submitted for the entire project prior to the start of any site work and construction. All debris materials proceeding from the project shall be put at the disposition of the Contractor and any salvaged materials will be dispensed for Contractors use. The Contractor shall utilize local contingency contracting for equipment and material to the maximum extent possible, but shall be prepared to import tools, equipment for demolition and construction (including light and heavy construction equipment), materials, and test equipment required to conduct the objective of this contract modification if these are not available in country. This project is unclassified and no USA security clearances are required.

2.1 The contractor shall conduct complete site and structural surveys that shall include at a minimum:

2.1.1 Topographic survey to mark the site’s exact location, dimensions and grade.

2.1.2 Power load analysis and recommendations of systems.

2.1.3 Analysis and recommendations for improvements to the Mechanical system.

2.1.4 Renovation and upgrades of physical conditions and deficiencies.

2.1.5 Civil/site work including inspection of existing utilities.

This project will required transportation of USG furnished material from the Canadian Embassy to Les Cayes CG station for modular building fabrication based on the attached inventory. Once units arrived to their final destination, the temporary power source (provided by contractor) within three days of delivery pending permanent connection to utilities.

2.2 Installation is expected for ten (10) modular buildings (GFE).

2.2.1 Modular Housing:

(a) Eight (8) x 20’ foot containers units transported to site under separate

CLIN

(b) Split HVAC units for climate control

2.2.2 Canadian – Ablution Unit

(a) One (1) 20’ foot container/Ablution Unit - container units transported to site.

2.2.3 Kennel

(a) One (1) x 20’ foot container (US GFE) converted into one (1) large open room (to be assembled).

(b) Split HVAC units for climate control

2.2.4 Storage

(a) One (1) x 20’ foot shipping container.

(b) Split HVAC installed for climate control.

(c) Multiple electrical outlets (per NEC requirements).

(d) Interior walls of storage area shall be lined with shelves and cabinets to preclude stacking supplies on the floor.

2.2.5 K9 Exercise Yard – Fenced in K9 Exercise Yard (40’ x 80’) with 40’x 10’ concrete pad. The contractor is expected to install a (6) foot chain link fence around the canine exercise yard including a 10’ x 50’ concrete Pad (see attached drawing.

2.2.5 Each container/pre-engineered building structure shall be elevated no less than two (2) feet from the ground level with structurally designed footers. These footers shall be re-locatable and re-usable (should he Compound need to be moved in the future). The footers shall firmly anchor the structures to the ground and sustain storm winds (120mph), etc. The personnel access steps constructed out of CMU Block and concrete mortar.

2.2.6 Contractor provides one (1) 20 foot shipping container.

2.3 Potable Water System

The contractor shall install a Potable Water System from existing water system at the facility. This system shall include a 3,000 gallon above ground reserve holding tank, associated pumping equipment, fixtures and plumbing.

2.4 Electrical Generator

The contractor shall perform the following:

2.4.1 Size, purchase and install an electric generator/system.

2.4.2 Verify operational capacity and load of generator.

2.4.3 Testing of generator will be accomplished in accordance with the manufacturer's written instructions or procedures.

2.4.4 The combined loads from multiple connections shall not exceed rated capacity of the standby generator.

2.4.5 Both the disconnect switch and the generator connection receptacle (s) must be installed within sight of the unit

2.4.6 Verify operation of automatic transfer switch

2.5 Install containment area for existing Above Ground Fuel Storage Tank:

2.5.1 Comply with EPA Federal AST Requirements under 40 CFR Part 112.

2.5.2 Ensure fuel tank is properly grounded.

2.5.3 Replace and install new piping, valves, etc. in compliance with American

Petroleum Institute standards

2.6 Connect water pump and modular buildings on BLTS site to the generator

2.6.1 Installation work shall comply with NEC 2008 Electrical Code.

2.6.2 Ensure well pump and accessories are supported by the generator (if required).

2.6.3 Ensure termination and connections comply with NEC 2008

2.7 Site Survey

The Contractor shall perform a comprehensive site survey that that envelopes a review of the property’s soil, vegetation, topography and hydrology in preparation for development of a sediment and erosion control plan for construction work performed on site.

2.8 Provide and install automatic transfer switch (ATS)

Technical performance needs to match or adapt future BLTS Headquarters construction enhancements. This technical work needs to be compatible with existing police facilities.

For this project, liquidated damages have been assesses in the amount of $1,314.00 US$191.60 per day for each day the work is not completed by the dated noted in the original terms of the contract.

3. CLIN 003: Sea Port – Port Au Prince Sea Port, Haiti

The Seaport site has an area with an existing compound as it’s indicated in the location pictures to be provided in the attachment section. This project includes, but it is not limited to site work, demolition of an existing building, and installation of pre-fabricated/modified office and housing containers, providing support utilities (electric, water, sewage) and installation of a six (6) chain link fence with triple strand barbed wire around the perimeter of the site (see conceptual layout attached). Site work will include clearing and cleaning existing area prior to footing installation for the two (2) US GFM (units) and one (1) pre-assembled unit (Canadian). One modular building to shall be in an office configuration (no toilet/shower) and the other Two (2) are in a double wide configuration (No toilet/shower). Electric systems shall be designed to support the three (3) modular buildings.

The Contractor shall be solely responsible for providing layout design concepts, procurement and installation of modified office/housing containers, construction services, utility and sewage connections, contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this contract within the established schedules, and in accordance with all of the main contract requirements.

Time is of the essence in the execution of this project and special attention will be placed on the safety practices of the Contractor. The Seaport, Port au Prince, Haiti is an active site. A safety plan must be submitted for the entire project prior to the start of any site work and construction.

All debris materials proceeding from the project shall be put at the disposition of the Contractor and any salvaged materials will be dispensed for Contractors use. The Contractor shall utilize local contingency contracting for equipment and material to the maximum extent possible, but shall be prepared to import tools, equipment for demolition and construction (including light and heavy construction equipment), materials, and test equipment required to conduct the objective of this contract modification if these are not available in country. The project is unclassified and no USA security clearances are required.

3.1 The Contractor shall conduct complete site and structural surveys that shall include at a minimum:

3.1.1 Topographic survey to mark the site’s exact location, dimensions and grade.

3.1.2 Renovation and upgrades of physical conditions and deficiencies.

3.1.3 Civil/site work including inspection of existing utilities.

3.2 General Requirements:

3.2.1 Two units are US supplied GFM and one (1) Canadian.

3.2.2 Foundations for Canadian Unit shall been in place prior to delivery from Port au

Prince

3.2.3 Canadian Unit shall be connected to temporary power source (provide by contractor) within three days of delivery pending permanent connection to utilities.

3.3 Special requirements

3.3.1 Single Dorm

a) One (1) x 20’ foot containers Canadian GFM.

b) Split HVAC units for climate control.

3.3.2 Office

a) Two (2) x 20’ foot container US GFM

b) Split HVAC units for climate control

3.3.3 Each container/pre-engineered building structure shall be elevated no less than two (2) feet from the ground level with structurally designed footers. These footers shall be re-locatable and re-usable (should the Compound need to be moved in the future). The footers shall firmly anchor the structures to the ground

3.4 Security Lighting

Install four (4) solar powered and weather proof perimeter security lights, three (3) with integral photo sensitive switch to illuminate the compound interior and perimeter at night.

Lights shall effectively illuminate the compound to preclude any dark areas within the compound.

3.5 Site Work

Coordinate all site work activities with the Airport Administrator including locations for temporary offices and material storage.

3.5.1 Septic system

Purchase and install all equipment/material necessary for a septic system that consists of septic tank. System to be designed to support the one modular building with toilet/shower/sink

3.5.2 Potable water system

The contractor shall install a Potable Water System to include water storage tank, pump and filtration system designed support the one modular building with toilet/sink/shower.

3.5.3 Perimeter fence

The contractor shall install a six (6) foot chain link fence around the perimeter of the yard (see attached drawing).

3.6 Electrical Generator/Electrical system

3.6.1 Size, purchase and install an electric generator/system (support for modular buildings only).

a) Verify operational capacity and load of generator.

b) Testing of generator will be accomplished in accordance with the manufacturer's written instructions or procedures.

c) The combined loads from multiple connections shall not exceed the rated capacity of the standby generator.

d) Both the disconnect switch and the generator connection receptacle(s) must be installed within sight of the unit.

e) Verify operation of automatic transfer switch

3.6.2 Install containment area for existing Above Ground Fuel Storage Tank.

a) Comply with EPA Federal AST Requirements under 40 CFR Part 112.

b) Ensure fuel tank is properly grounded.

c) Replace and install new piping, valves, etc. in compliance with American

Petroleum Institute standards,

3.6.3 Connect modular buildings on the BLTS site to the Generator.

a) Installation work shall comply with NEC 2008 Electrical Code.

b) Ensure well pump and accessories are supported by the generator (if required).

c) Ensure termination and connections comply with NEC 2008.

4. CLIN 004: Malpasse Border Post – Malpasse Commissariat, Haiti

This project location is at the BLTS facilities at the Malpasse Commissariat, Malepasse. The contractor will install two (2) US GFM units. The Malpasse Commissariat has a parking area next to the station (see J-2-04 INL Project Conceptual Details/Drawings). This project includes, but it is not limited to site work, and installation of two (2) units double wide configuration and installation of a six (6) chain link fence with triple strand barbed wire around the perimeter of the site (see J-2-04 INL Project Conceptual Details/Drawings). Site work will include clearing and cleaning existing area prior to footing installation. The doublewide building will be open space for storage with one desk and chairs. Electric systems shall be designed to support both units.

Additional work includes the renovation of the existing Commissariat (water/sewer/electric).

The Contractor shall be solely responsible for providing layout design concepts, procurement and installation of modified office/housing containers, construction services, utility and sewage connections, contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this task order modification within the established schedules, and in accordance with all of the main contract requirements.

Time is of the essence in the execution of this project and special attention will be placed on the safety practices of the Contractor. The Terrier Rouge Customs Station is an active station. A safety plan must be submitted for the entire project prior to the start of any site work and construction. All debris materials proceeding from the project shall be put at the disposition of the Contractor and any salvaged materials will be dispensed for Contractors use. The Contractor shall utilize local contingency contracting for equipment and material to the maximum extent possible, but shall be prepared to import tools, equipment for demolition and construction (including light and heavy construction equipment), materials, and test equipment required to conduct the objective of this contract modification if these are not available in country. This project is unclassified and no USA security clearances are required.

The Contractor shall conduct complete site and structural surveys that shall include at a minimum:

4.1 General Requirement

4.1.1 Building Renovation

a) Renovate existing Commissariat building – Electric/water/sewer

b) Office/Storage

c) Two (2) x 20’ foot containers (double wide configuration) converted into one

(1) open room.

4.1.2 Split HVAC units for climate control

4.1.3 Each container/pre-engineered building structure shall be elevated no less than footers shall be re-locatable and re-usable (should the Compound need to be moved in the future). The footers shall firmly anchor the structures to the ground

4.2 Site Work

4.2.1 Security Lighting

Install four (4) solar powered and weather proof perimeter security lights (See attachment 3) with integral photo sensitive switch to illuminate the compound interior and perimeter at night. Lights shall effectively illuminate the compound to preclude any dark areas within the compound.

4.2.2 Septic System

Renovate existing septic system on the current Mallpasse Commissarit Building

4.2.3 Potable Water System

Renovate existing water system on the current Mallpasse Commissariat Building

4.2.4 Perimeter fence

The contractor to install a six (6) foot chain link fence around the perimeter of the double wide configuration. Fence posts to be installed in existing parking area by cutting out existing material and excavating for fence post footings.

4.3 Electrical Generator/Electrical System

4.3.1 Size, provide and install electric generator/system

a) Verify operational capacity and load of generator.

b) Testing of generator will be accomplished in accordance with the manufacturer's written instructions or procedures.

c) The combined loads from multiple connections shall not exceed the rated capacity of the standby generator.

d) Both the disconnect switch and the generator connection receptacle(s) must be installed within sight of the unit.

4.3.2 Install containment area for existing Above Ground Fuel Storage Tank.

b) Ensure fuel tank is properly grounded.

4.3.3 Connect the modular buildings on the BLTS site to the Generator.

b) Ensure well pump and accessories are supported by the generator (if required).

5. CLIN 005: Terrier Rouge Customs-Terrier Rouge, Haiti

The Terrier Rouge Customs Station has a concrete parking area behind the station (see J-2-04 INL Project Conceptual Details/Drawings). This project includes, but it is not limited to site work, and installation of four (4) Canadian pre-fabricated units consisting of: office (1), housing (2), and washroom container (1), providing support utilities (electric, water, sewage), installation of one (1) incinerator, and installation of a six (6) chain link fence with triple strand barbed wire around the perimeter of the site (see J-2-04 INL Project Conceptual Details/Drawings). Site work will include clearing and cleaning existing area prior to footing installation for the four (4) GFE (Canadian). One modular building shall be in an office configuration (no toilet/shower), one in a Dorm configuration (no toilet/shower), and the other a washroom modular. Water/sewer/electric systems shall be designed to support the four (4) modular buildings.

The Contractor shall be solely responsible for providing layout design concepts, procurement and installation of modified office/housing containers, construction services, utility and sewage connections, contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this task order modification within the established schedules, and in accordance with all of the main contract requirements.

5.1 General Requirements

All modular units for this project will be provided by the USG based on attached list of inventory. The following considerations are also required:

5.1.1 Foundations for Canadian Units shall been in place prior to delivery from Port au

Prince

5.1.2 Canadian Units to be connect to temporary power source (provide by contractor) within three days of delivery pending permanent connection to utilities.

5.2 Housing development

5.2.1 Single Dorm

(a) Two (2) x 20’ foot containers GFM (Canadian).

(b) Split HVAC units for climate control

5.2.2 Office

(a) One (1) x 20’ foot container GFM (Canadian)

(b) Split HVAC units for climate control.

5.2.3 Ablution Unit

(a) One (1) x 20’ foot GFM (Canadian)

(b) Split HVAC units for climate control existing

5.2.4 Incinerator

(a) Transport from Port au Prince Airport.

(b) Install one (1) incinerator

5.2.5 Each container/pre-engineered building structure shall be elevated no less than two (2) feet from the ground level with structurally designed footers. These footers shall be re-locatable and re-usable (should the Compound need to be moved in the future). The footers shall firmly anchor the structures to the ground

5.3 Septic System

Purchase and install all equipment/material necessary for a septic system that consists of septic tank and soak pit. The system is expected to support the one modular building with toilet/shower/sink.

5.4 Portable Water System

The contractor shall install a Potable Water System to include water storage tank, pump and filtration system designed support the one modular building with toilet/sink/shower.

5.5 Perimeter Fence

The contractor to install a six (6) foot chain link fence around the perimeter of the yard (See J-2-04 INL Project Conceptual Details/Drawings). Fence posts to be installed in existing concrete pad by cutting out the concrete and excavating for fence post footings.

5.6 Electrical Generator/Electric system

5.6.1 The contractor shall perform the following:

(a) Size, provide and Install electrical generator and associated equipment.

(b) Verify operational capacity and load of generator.

(c) Testing of generator will be accomplished in accordance with the manufacturer's written instructions or procedures.

(d) The combined loads from multiple connections shall not exceed the rated capacity of the standby generator.

(e) Provide and install inverter system including batteries to support required electric load.

(f) Both the disconnect switch and the generator connection receptacle(s) must

5.6.2 Install containment area for existing Above Ground Fuel Storage Tank.

(a) Comply with EPA Federal AST Requirements under 40 CFR Part 112.

(b) Ensure fuel tank is properly grounded.

(c) Replace and install new piping, valves, etc. in compliance with American

5.6.3 Connect the modular buildings and water pump on the BLTS site to the Generator.

(a) Installation work shall comply with NEC 2008 Electrical Code.

(b) Ensure well pump and accessories are supported by the generator (if required).

(c) Ensure termination and connections comply with NEC 2008.

(d) Install additional electrical connection to existing Customs Building electrical panel.

6. CLIN 006: Central Directorate of Police Justice (DCPJ)-MINUSTA PAP Airport Compound, Haiti

This Scope of work is for the construction of the DCPJ facilities at the MINUSTA Compound, Port au Prince, Haiti. The DCPJ site has an area available (See J-2-04 INL Project Conceptual Details/Drawings). This project includes, but it is not limited to site work, and installation of four

(4) pre-fabricated office and housing containers, connecting to support utilities (electric and IT).

The four (4) units are GFM (Canadian).

The Contractor shall be solely responsible for providing layout design concepts, procurement and installation of modified office/housing containers, construction services, utility and contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this task order modification within the established schedules, and in accordance with all of the main contract requirements.

Time is of the essence in the execution of this project and special attention will be placed on the safety practices of the Contractor. The Seaport, Port au Prince, Haiti is an active site. A safety plan must be submitted for the entire project prior to the start of any site work and construction.

All debris materials proceeding from the project shall be put at the disposition of the Contractor and any salvaged materials will be dispensed for Contractors use.

The Contractor shall utilize local contingency contracting for equipment and material to the maximum extent possible, but shall be prepared to import tools, equipment for demolition and construction (including light and heavy construction equipment), materials, and test equipment required to conduct the objective of this contract modification if these are not available in country. This project is unclassified and no USA security clearances are required. The Contractor shall conduct complete site and structural surveys that shall include at a minimum:

(a) Topographic survey to mark the site’s exact location, dimensions and grade.

(b) Renovation and upgrades of physical conditions and deficiencies.

(c) Civil/site work including inspection of existing utilities.

6.1 General Requirements

6.1.1 Foundations for Canadian Units shall been in place prior to delivery from Port au Prince

6.1.3 Provide and install conduit for electric connection to main building.

6.1.4 Provide and install conduit for IT connection to main building

Note: For the installation of the conduit in the items above a trench in a concrete driveway will need to be cut and concrete repaired once conduit is installed.

6.1.5 Office

a) Two (2) x 20’ foot containers Canadian GFE

b) Split HVAC units for climate control.

6.1.6 Each container/pre-engineered building structure shall be elevated no less than footers shall be re-locatable and re-usable (should the Compound need to be moved in the future). The footers shall firmly anchor the structures to the ground

6.2 Electrical System

6.2.1 Provide and install electrical conduit to connect to main building electric system

6.2.2 Provide and install additional battery rack with 12 deep cell batteries for electric inverter system. Batteries to be equal to existing batteries.

6.2.3 Connect all connect two (2) modular buildings to the electric system.

b) Ensure termination and connections comply with NEC 2008.

7. CLIN 007: PAP City Haul – Port Au Prince, Haiti

Transportation service from the Canadian Embassy of one (1) modular unit to PAP City Haul – Port au Prince.

8. CLIN 008-Cap Haitian Coast Guard Base for BLTS, Haiti

For this project, two (2) US GFM will be provided, details are found in the US Modular inventory list (See J-2-04 INL Project Conceptual Details/Drawings). This project exact location is at the Cap Haitian Station behind CG station. This project includes, but it is not limited to site work, and installation of pre-fabricated/modified office and housing containers, connection to existing utilities (electric, water, sewage) and renovation of the existing building’s electric system. Site work will include clearing, grading and compacting existing area prior to footing installation for the two (2) units in a double wide configuration.

The main building at the Cap Haitian Coast Guard facility will require a complete overhaul of electrical and plumbing. With the addition of the BLTS Modular unit in the rear lot of the complex, the water, plumbing, septic, and electrical network on-site will require a total evaluation and adjustment.

The Contractor shall conduct complete site and structural surveys that shall include at a

8.1 The contractor is expected to perform as follows:

8.1.1 Double wide Office to include,

(a) Two (2) x 40’ foot containers converted into one (1) large open room.

(b) Split HVAC units for climate control.

(c) Unit shall contain one (1) full bathroom (shower, toilet, sink) (GFE). No hot water.

8.1.1 CG Facility building renovation requirements

(a) Evaluate the entire electrical system, to include generators, transformer, manual transfer switch, service line, service head, panel box, and existing electrical condition in main Coast Guard Building.

(b) Remove and replace/install all defective electrical components, and replace with new to include lights, outlets (interior/exterior), switches, ceiling fans, AC units, panel box, service line, service head, and ventilation fans. All conduits will be exterior (EMT) surface mounted. In addition add: (1) Electrical line and waterproof outlet from main building to ramp area for future electrical requirements. (2) Install two (2) marine plugs at dock ramp walkway.

(c) Evaluate the entire plumbing system, to include water tank storage, water pump, pressure tank, water lines, sinks, toilets, drainage, septic system, showers, plumbing fixtures, shower dividers/curtains, and wash basins.

(d) Remove and replace/install all defective plumbing fixtures, water pumps, water lines, toilets, sinks, pressure switches, pressure tanks, septic system/tanks (service or replace), wash down area, and wash basins. In addition add waterline/valve from main building to boat ramp area for future wash down or water requirements.

(e) Pressure Wash all exterior surfaces on main building. Repair any damaged stucco, wood, broken windows, railing. Paint all exterior surfaces with flat latex base paint- match existing colors. Paint all hand rails with enamel based paint-match existing colors.

(f) Install #10 ABC Wall mounted Fire Extinguisher in main entrance room.

(g) Install Smoke Detectors (Battery Operated) in all rooms. Meet or exceed

NFPA-72 Standards.

(h) Paint interior wall with three coats of semi-gloss paint

8.2 Each container/pre-engineered building structure shall be elevated no less than two

(2) feet from the ground level with structurally designed footers. These footers shall be re-locatable and re-usable (should the Compound need to be moved in the future).

The footers shall firmly anchor the structures to the ground and sustain storm winds (120mph), etc. The personnel access steps constructed out of CMU Block and concrete mortar.

8.3 Site Work

8.3.1 Septic System

(a) Contractor shall install a septic system that includes the existing septic tank.

(b) Contractor shall pump out the existing septic tank.

8.3.2 Potable Water System

The contractor shall install a Potable Water System from existing water system to the facility. This system shall include pumping equipment, fixtures and plumbing.

The water well shall provide potable water and be low maintenance.

8.4 Electrical generator: The contractor shall perform the following:

8.4.1 Perform a technical evaluation of the electrical generator requirement.

8.4.2 Verify operational capacity and load of generator.

8.4.3 Testing of generator will be accomplished in accordance with the manufacturer's written instructions or procedures.

8.4.4 The combined loads from multiple connections shall not exceed the rated capacity of the standby generator.

8.4.5 Both the disconnect switch and the generator connection receptacle(s) must be installed within sight of the unit.

8.4.6 Verify operation of automatic transfer switch

8.5 Install containment area for existing above ground fuel storage tank.

8.5.1 Comply with EPA Federal AST Requirements under 40 CFR Part 112

8.5.2 Ensure fuel tank is properly grounded.

8.5.3 Replace and install new piping, valves, etc. in compliance with American

Petroleum Institute standards,

8.6 Connect modular units on the BLTS site to the generator.

8.6.1 Installation work shall comply with NEC 2008 Electrical Code.

8.6.2 Ensure well pump and accessories are supported by the generator (if required).

8.6.3 Ensure termination and connections comply with NEC 2008.

8.7 Provide and install automatic transfer switch (ATS)

9. CLIN 009: US Embassy- Tabare 41, Port Au Prince, Haiti

The US Embassy has an existing warehousing area across the street for the main embassy compound (See J-2-04 INL Project Conceptual Details/Drawings). This project includes, but it is not limited to site work, and installation of pre-fabricated/modified office, connection to existing utilities (electric), Site work will include clearing and cleaning existing area prior to footing installation for the two (2) US GFM. Modular buildings shall be constructed in a “double wide” configuration.

9.2 Electrical Generator/Electrical system

The contractor shall purchase and install electrical conduit (PVC), electrical breaker and required electrical wiring in order to connect to existing electrical system at the site.

10. CLIN 010: BLTS PAP IAP K9, Haiti

The Port au Prince K9 Facility has an existing concrete pad housing dog kennels feet based on picture provided in attached documents. Contractor shall extend existing pad and concrete pad shall be 12’ X 60’. The Contractor shall be solely responsible for providing layout design concepts, contract coordination, supervision, including but not limited to the management, professional services, design services, all labor, material, and equipment necessary to meet the requirements of this task order modification within the established schedules, and in accordance with all of the main contract requirements.

A safety plan must be submitted for the entire project prior to the start of any site work and construction. All debris materials proceeding from the project shall be put at the disposition of the Contractor and any salvaged materials will be dispensed for Contractors use.

The Contractor shall utilize local contingency contracting for equipment and material to the maximum extent possible, but shall be prepared to import tools, equipment for demolition and construction (including light and heavy construction equipment), materials, and test equipment required to conduct the objective of this contract modification if these are not available in country. This project is unclassified and no USA security clearances are required.

The Contractor shall conduct complete site and structural surveys that shall include at a minimum:

The Seaport site has an area with an existing seaport area as it’s indicated in the location pictures to be provided in the attachment section. This project includes, but it is not limited to site work, demolition of an existing building, and installation of pre-fabricated/modified office and housing containers, providing support utilities (electric, water) and installation of a six (6) foot chain link fence with triple strand barbed wire around the perimeter of the site (see conceptual layout attached).

Site work will include clearing and cleaning existing area prior to footing installation for the three (3) US GFM (units). One modular building to shall be in an office configuration (no toilet/shower) and the other Two (2) are in a double wide configuration (No toilet/shower).

Electric systems shall be designed to support the three (3) modular buildings.
