About this file
This document is a State of Oklahoma Office of Management & Enterprise Services (OMES) Capital Assets Management Contractor Invoice form (CAP Form G702), specifically a sample invoice template for contractors working on state construction projects. The form is designed for contractors to submit payment applications, with sections for detailing work completed, materials stored, and certification of work progress. The invoice includes multiple sections such as a Contractor's Invoice and Certification, Owner's Representative Certification, and a detailed Continuation - Schedule of Values.
The form provides a standardized mechanism for contractors to request payment by documenting completed work, stored materials, and project progress. It includes provisions for retainage (5% in this template), tracking of scheduled values, work completed, and balance to finish. The document is part of Oklahoma's state procurement process, specifically for construction and property management projects, and is modified from an AIA (American Institute of Architects) standard document. The form requires detailed documentation and certification by both the contractor and the owner's representative, ensuring transparency and accountability in the payment process for state-funded construction projects.

The following is an example of a Continuation Sheet for work in progress, Please note that dollar amounts shown below are for illustrative purposes only, and are not intended to reflect actual construction costs.
Column Excluded: An "x" should be placed within this column if the line item is exempt from retainage, i.e. phase underwent Substantial Completion, end of Preconstruction (CMc) phase billing.
Change Orders: Although Change Orders could be incorporated by changing the schedule of values each time a Change Order is added to the Project, this is not the State's method.Change Orders are to be listed separately at the end of the basic schedule. The amount of the original contract adjusted by Change Orders is to be entered in the appropriate location on the G702 form.

Only as materials are incorporated into the Project is their value deducted from this column and incorporated into column E (Work Completed-This Period.)
