About this file
This is a Past Performance Questionnaire (PPQ) template used to evaluate contractor performance on completed federal contracts. The form is designed to be completed by both contractors and clients to document and assess work quality, schedule compliance, cost management, and overall performance on government projects.
The questionnaire is divided into two main sections: contractor information (Blocks 1-4) and client information (Blocks 5-8). Contractors must provide firm details, contract information including contract number, type, dates, and pricing, plus a project description with complexity level. Clients complete the evaluation by rating contractor performance across seven key categories: Quality, Schedule/Timeliness of Performance, Customer Satisfaction, Management/Personnel/Labor, Cost/Financial Management, Safety/Security, and General performance. Each category contains multiple sub-elements rated on a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). The questionnaire also includes Yes/No questions on contract terminations, financial problems, and contractor rehire likelihood. Clients provide narrative comments addressing strengths, weaknesses, and deficiencies. Per the instructions, VA requests clients submit completed questionnaires directly to the offeror, who may duplicate and submit with proposals for future solicitations, though questionnaires may also be submitted directly to VA. The government reserves the right to verify all information provided.
