About this file

This is a Past Performance Client Questionnaire template designed for contractors to obtain references from previous clients regarding their performance on past contracts. The questionnaire will be submitted to the Department of Veterans Affairs, Network Contracting Office 12 (NCO 12) in Milwaukee, Wisconsin, as part of the source selection evaluation process for solicitation 36C25226R0038, a 100% small-business set-aside for IDIQ ACM Abatement Services at Hines VA Hospital in Hines, Illinois. Completed questionnaires should be emailed directly to Kristi.Kluck3@va.gov, and responses will be kept confidential with respondent names withheld from the contractor.

The questionnaire requires clients to rate contractor performance across seven evaluation categories using a five-point scale: Exceptional (E), Good (G), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Neutral/Not Applicable (N). The categories assessed include quality of technical work and problem resolution; schedule and timeliness compliance; customer satisfaction and cooperation; management effectiveness, personnel qualifications, and small business utilization; cost and financial management including invoice accuracy and budget compliance; safety and security compliance; and general responsiveness to emergencies and contractual adherence. The questionnaire requests narrative comments addressing specific strengths, weaknesses, deficiencies, and overall performance risk, with clients also indicating whether they would hire the contractor again and providing an overall performance rating.
