Statement of Work Summary
This is a Statement of Work for Project 695-22-123, prepared by the Clement J. Zablocki VA Medical Center (Department of Veterans Affairs) in Milwaukee, Wisconsin. The project involves upgrading the North Campus medium voltage electrical distribution system through comprehensive construction services. The contractor must furnish all labor, materials, equipment, qualified supervision, and items specified in the contract to convert the existing radial electrical distribution system to a composite primary loop radial system, including addition and replacement of pad-mounted distribution equipment, underground medium voltage feeders, duct banks, and kirk-key interlock systems. Additional scope includes current transducer fault indicator replacements, detailed project phasing to minimize impacts to medical center operations, and comprehensive utility survey services with precise geographic location and depth documentation.
The project encompasses seven phases spanning from preliminary work through post-substantial completion. Phase 0 requires submission of long lead time equipment and material submittals with a minimum 60 calendar days for VA review and approval; once ordered, the project enters suspension to preserve contract days. Upon suspension lift and establishment of a construction mobilization date, the VA will issue a Notice to Proceed (NTP). Phases 1-3 comprise pre-construction activities with specific deadlines (NTP+30, NTP+60, and NTP+90 calendar days respectively) requiring construction schedules, safety plans, security plans, phasing plans, and various certifications. Phase 4 allows 245 calendar days for construction (NTP+335 deadline), and Phase 5 provides 75 calendar days for post-substantial completion activities (NTP+410 deadline). All construction must be completed within a single calendar year with no overlap into multiple years, and the contractor must deliver as-built drawings, utility survey reports, warranties, operation and maintenance manuals, and staff training within 45 days of substantial completion.
