About this file
This is a Certified Disadvantaged Business Outreach Plan attachment for a Price Agreement procurement issued by the State of Oregon, acting through the Oregon Health Authority, Oregon State Hospital, for the purchase of patient mattresses on an as-needed basis. The form requires bidders to provide contact information and certify their compliance with Oregon's requirement to ensure Certified Firms—small businesses certified by the Oregon Certification Office for Business Inclusion and Diversity (COBID) as minority-owned, woman-owned, veteran-owned, or emerging small businesses—receive equal opportunities to participate in subcontracting. Bidders must indicate their own certification status if applicable, list prior contractual relationships with Certified Firms from the past two years, and identify non-certified firms that may qualify as minority-owned, woman-owned, service-disabled veteran-owned, or emerging small businesses with which they have worked.
For procurements with anticipated subcontracting opportunities, bidders must describe the specific steps they will take to solicit Certified Firms, identify and estimate the dollar values of available subcontracting opportunities, and confirm their willingness to report subcontractor identity and values to COBID upon award. The outreach plan is mandatory for bidders anticipating subcontract work but optional for those with no subcontracting opportunities. Completion and submission of all applicable pages with the bid is required. This outreach plan does not factor into bid evaluation but serves to document the bidder's commitment to providing Certified Firms with meaningful participation opportunities in the contract's performance.
