About this file

This document is a Request for Quotation (RFQ) issued by the West Virginia Office of Shared Administration (OSA) Purchasing Division on behalf of the Department of Health Facilities (DHF) for facility equipment maintenance at William R Sharpe Jr. Hospital in Weston, West Virginia. The RFQ (WSH2500000002) seeks to establish an open-end contract for preventive and corrective maintenance services for facility equipment. The contract will cover maintenance activities for specified equipment, with services to be performed by technicians with at least five years of experience and CFESA Master Certification. Maintenance services include both scheduled preventive maintenance and as-needed corrective maintenance, with a 48-hour response time for service calls and work to be completed between 7:00 AM and 4:00 PM, Monday through Friday.

The contract will be awarded to the vendor providing the lowest total cost, with pricing structured to include monthly preventative maintenance, hourly rates for corrective maintenance, parts markup, and trip charges. The contract requires advanced approval for parts over $500, mandates that replacement parts be equal or better than original manufacturer parts, and includes a one-year warranty on installed parts. Notably, the contract may be funded partially or wholly with federal funds and is subject to federal funding requirements. The vendor is responsible for all travel costs, must provide their own tools and equipment, and will be held to strict performance standards with potential immediate contract cancellation for non-compliance.
