About this file
This is a Request for Qualifications (RFQ) issued by the State of Colorado Department of Public Health and Environment (CDPHE) Hazardous Materials and Waste Management Division seeking qualified firms to provide lead remediation services for the Lake County Community Health Program (LCCHP). The RFQ solicits Statements of Qualifications from contractors experienced in residential lead abatement to be placed on a qualified contractor list for notification of future project opportunities. The scope of work includes implementing lead abatement site plans at residential properties in the Leadville area, developing project schedules and costs, coordinating with CDPHE staff and property owners, obtaining necessary permits, performing remediation work, disposing of waste, and restoring project sites to conditions meeting lead abatement requirements. Statements of Qualifications were due June 5, 2017 at 2:00 PM, with acceptance notifications expected June 9, 2017. Qualified contractors would be accepted for a maximum period of five years from acceptance, after which they would be notified of individual projects available for bid. The RFQ included an inquiry deadline of May 26, 2017 and posted answers on May 30, 2017. Due to Leadville's short construction season, remediation services must typically commence no earlier than June 1 and be completed by September 30.
Evaluation of qualifications occurs on a pass/fail basis, with acceptance contingent upon demonstrated successful completion of at least three previous residential lead abatement projects, OSHA 40-hour HAZWOPER training for all staff, accredited lead-based paint abatement worker training for personnel performing dust abatement and lead-based paint remediation activities, accredited supervisor training for supervisors, and availability of a state-certified lead-based paint abatement supervisor on staff. Contractors must demonstrate adequate staffing to provide timely services and disclose any financial interests in Lake County properties to identify potential conflicts of interest. Selected contractors must maintain Workers' Compensation Insurance, Commercial General Liability Insurance with minimum limits of $1,000,000 per occurrence and aggregate, and Automobile Liability Insurance with combined single limits of $1,000,000, with the State of Colorado named as additional insured. Subsequent project awards will be based on documented quotes from qualified contractors, with consideration of price, proposed materials, and ability to meet project schedules. No pricing information is contained in this qualification phase document.
