About this file

This is a Past Performance Information Sheet template for offerors responding to solicitation FA440726QBM01 for Fire Suppression maintenance services with the Department of the Air Force Air Mobility Command. Offerors must complete one form for each of three required past performance references, with all references required to be recent (work performed within five years of the RFQ issuance date) and relevant to maintenance, repair, and installation of fire suppression systems. The form requests general project information including title, contract/order number, and performing company designation; customer and agency details with primary point of contact information and willingness to participate in performance interviews; contract specifics such as total value and period of performance; and a detailed narrative describing work performed with explicit connections to the RFQ's Performance Work Statement, including scope, magnitude, and complexity of the effort.

The template also requires offerors to provide evidence of performance quality through CPARS records (if available) with ratings in key evaluation areas such as Quality, Schedule, Cost Control, and Management; letters of recommendation; or other performance assessments such as customer surveys. By submitting the past performance information, offerors grant the U.S. Government permission to contact the listed customer points of contact to verify the provided information and inquire about performance on the cited contracts. This template is a standard evaluation document for assessing contractor capability and past performance on similar fire suppression system maintenance projects.
