About this file
This is a Reference Check Form attachment for RFP #S-63500-00015834 issued by the Deschutes and Mid-Columbia Wildlife Districts for helicopter services in Oregon. The form establishes the evaluation criteria for assessing proposer qualifications through professional references. Proposers are required to submit three completed Reference Check Forms from current professional references, with at least two references from non-Oregon Department of Fish and Wildlife (ODFW) sources and one from ODFW or a different firm/client. Each reference submission must include the reference entity name, contact person, telephone number, and email address, with the understanding that the Agency will attempt to contact each reference up to three times to gather evaluation data.
The reference evaluation process allocates a total of 15 possible points across six assessment questions rated on a 0-5 scale, where 0 indicates not satisfied, 2.5 indicates moderately satisfied, and 5 indicates extremely satisfied. The evaluation criteria focus on overall quality of services and proposer relationship, safety performance, responsiveness to customer service issues, pilot flexibility and communication regarding survey-related changes and feedback, experience and capability in wildlife survey projects in remote locations, and likelihood of continued contractor engagement for future projects. Proposers who fail to provide contactable references will receive a score of zero for that reference.
