SUBJECT: Amendment No. Five (5) to Request for Proposal (RFP) 294-12-000008;

Local Construction Program (LCP)

Amendment Date: November 14, 2012

Closing Date: November 26, 2012

Closing Time: 16:00, Tel Aviv Local Time

ATTENTION PROSPECTIVE OFFERORS:

The purpose of this Amendment is to advise all prospective offerors of the following:

1. Under the RFP, Section M.2, Technical Evaluation Criteria, increase the number of points allocated to the Offeror’s Experience criterion from 20 to 25 points and decrease the number of points allocated to the Past Performance criterion from 15 to 10 points as follows:

1.A Delete the table under Section M.2 in its entirety and replace it with the following:

1.B Under Section M.2, page M-2, delete bullets “I” through “V” in their entirety and replace them with the following:

“I. Offeror’s Capacity– Basic IQC (35 points)

Extent to which the Offeror convincingly demonstrates that it has the current capacity to effectively implement several large scale infrastructure construction projects concurrently in conformity with international best practice in the construction industry in one or more of the sectors described in Sections C.8.1.a through C.8.1.d.

II. Offeror’s Experience– Basic IQC (25 points)

Extent to which the Offeror’s experience convincingly demonstrates its ability to implement individual and multiple task orders under this program in one or more of the sectors described in Sections C.8.1.a through C.8.1.d.

III. Gender Action Plan - Basic IQC ( 5 points)

The extent to which the Offeror proposes a Gender Action Plan that convincingly demonstrates the Offeror’s commitment to the promotion of gender equality and the achievement of the objectives outlined in Section

C.9.11.

IV. Key Personnel – Basic IQC (10 points)

The extent to which the proposed IQC Program Director possesses the qualifications and demonstrated past performance to successfully lead the implementation of the IQC.

V. Past Performance – Basic IQC (10 points)

Extent to which successful past performance is evidenced in one or more of the sectors described in Sections C.8.1.a through C.8.1.d.”

2. Under Attachment J-6-A.1, Section F.7, Insurance, add the following:

“Insurance Ceiling:

1. Commercial General Liability Insurance Ceiling:

For persons other than the contractor's employees the insurance shall be at a minimum for $250,000 per case and not to exceed $1,500,000. For damage to property of USAID or others arising out of any act or omission of the

Contractor or its agents, employees, or subcontractors the insurance shall be at a minimum for $500,000 per occurrence.

2. Commercial Automobile Liability Insurance Ceiling:

This insurance depends on the type of vehicle.

3. Subcontractor's Public Liability and Property Damage Insurance and

Vehicle Liability Insurance ceiling:

For public injuries the insurance shall be at a minimum for $150,000 per case and not to exceed $900,000. For damage to property insurance, at a minimum, shall be $250,000 per occurrence.

4. Builder's Risk Insurance Ceiling:

The amount of such insurance shall be no less than the insurable value of the work at completion plus equipment.”

3. Answer questions received under Round II, attached to this amendment.

There are no additional changes to this RFP.
