About this file

This is a Responsibility Determination Form issued by the Oregon Department of Administrative Services (DAS) State Procurement Services for use in evaluating proposer qualifications prior to contract award. The form applies to ITB No. S-10700-00012203 for the Supply of Bulk Propane Fuel, Tanks, Related Goods and Services. The document serves as a standardized responsibility inquiry tool to assess whether proposers possess the necessary financial, material, equipment, facility, and personnel resources to meet all contractual obligations.

The form requires proposers to provide detailed responses across eight key evaluation areas: availability of resources and expertise to perform contract work; historical performance on similar contracts within the past five years, specifically regarding adherence to time and budget constraints; disclosure of any criminal or civil charges, indictments, or convictions within the past three years related to public contracts, antitrust violations, or fraud; documentation of contract terminations for default or litigation within the past three years; status of outstanding judgments and financial distress; bankruptcy or insolvency filings within the past three years; possession of required licenses, insurance, registrations, and legal authorization to conduct business in Oregon; and, if applicable, submission of a current Pay Equity Certificate for firms employing 50 or more full-time workers on contracts exceeding $500,000. Proposers must certify under penalty that all responses are complete, accurate, and not misleading, with an authorized representative's signature required on the form.
