About this file

This Amendment No. 1, dated April 28, 2023, to RFP No. F-23-083/MD issued by Palm Beach County Purchasing Department pertains to the pre-qualification of providers for in-home services under the direction of the Florida Department of Elder Affairs through the Area Agency on Aging of Palm Beach/Treasure Coast, Inc. The amendment revises Appendix A (Price Proposal Pages) by removing lines ten and eleven from all three pricing tables and renumbering subsequent lines accordingly. The RFP seeks qualified providers to deliver personal care, respite care, homemaker services, companionship, chore services, enhanced chore services, medication management, and gerontological and mental health/screening counseling services across three geographic lots: Lot #1 (Countywide, excluding North Palm Beach County and the Glades Area), Lot #2 (Glades Area), and Lot #3 (North Palm Beach County). The original proposal submission deadline was May 26, 2023. The initial contract term is twelve months with four subsequent twelve-month renewal options. Proposers must acknowledge receipt of this amendment by signing and returning the acknowledgment page with their proposal response, or the proposal will be deemed non-responsive.

Pricing for all services must be submitted as fully-burdened hourly rates, including all overhead, general, administrative costs, and profit, with unit prices limited to two decimal points. Price adjustment requests during renewal periods are permitted but cannot exceed the published Consumer Price Index rate or three percent, whichever is less, and must include substantial supporting documentation. The contract is federally funded and subject to compliance with multiple federal and state certifications and assurances, including nondiscrimination provisions, debarment certification, lobbying restrictions, background screening requirements under Florida Statutes Chapter 435, HIPAA compliance, and E-Verify employment verification. Vendors are required to maintain financial management systems capable of accurate grant reporting, conduct audits if expending $750,000 or more in federal awards annually, and comply with all applicable civil rights and compliance requirements. The contract includes provisions for monitoring, on-site visits, and audit procedures by the Department of Elder Affairs. Billing procedures require vendors to submit invoices electronically on a bi-weekly basis through the Palm Beach County Vendor Portal, with services invoiced in one-hour increments and payment contingent upon actual service delivery in clients' homes.
