About this file

This is a Request for Proposal (RFP) document issued by the South Dakota Department of Health, Office of Chronic Disease Prevention and Health Promotion (OCDPHP), for Chronic Disease Management Programming as part of the Rural Health Transformation initiative. The RFP seeks health organizations to implement evidence-based chronic disease management activities and remote patient monitoring programs addressing leading causes of death and disability in South Dakota's rural communities. The procurement comprises two components: Component 1 focuses on chronic disease evidence-based interventions including care coordination, community health worker programs, blood pressure monitoring, cancer screening, diabetes self-management education, pharmacist-led medication management, and condition-specific interventions; Component 2 provides funding for remote patient monitoring equipment purchases such as blood pressure cuffs, glucometers, and pulse oximeters. Eligible applicants include South Dakota clinics, hospitals, pharmacies, dental clinics, tribal health facilities, and Indian Health Services entities, with preference given to organizations in and serving rural and frontier communities. The RFP was published on March 30, 2026, with offeror questions due by April 17, 2026, and state responses provided by April 22, 2026. Proposals must be submitted electronically to Laura.Streich@state.sd.us by May 14, 2026, at 5:00 PM CST, with an anticipated award decision on June 19, 2026. The contract term runs from July 1, 2026, through October 30, 2027, with an option to renew for three additional one-year periods.

The total financial assistance award is $189,477,607.26 across all participants, with individual award values capped at $150,000 per 12-month year; year one budgets are extended to 16 months and capped at $200,000. Component 2 equipment purchases are limited to no more than $50,000 over the entire four-year funding period. The project is 100% funded by the Centers for Medicare & Medicaid Services/Health and Human Services. Allowable costs include personnel (salary and fringe benefits), supplies, travel at state per diem rates, remote patient monitoring equipment, and pre-approved conferences and trainings. Indirect costs may be included for executive oversight, accounting, and contract management, not to exceed the federally approved indirect cost rate of 6.3% of the total contract amount. Unallowable costs include gift cards, food, incentives, research activities, lobbying, out-of-state travel, construction or renovation, and permanent equipment unrelated to remote patient monitoring. No carry-over funding is permitted. Applicants must submit monthly progress reports and itemized invoices, annual reports with performance measures and success stories, and participate in Rural Health Transformation evaluation activities. There are no identified incumbent contractors. Evaluation criteria prioritize technical qualifications over cost, including specialized expertise, resources, past performance, project management techniques, and personnel qualifications.
