About this file
Attachment B is a Project Narrative Form for the Chronic Disease Management Programming Request for Proposal #26-09RHT-002, administered by the South Dakota Department of Health, Office of Chronic Disease Prevention and Health Promotion. The form requires applicants to describe evidence-based chronic disease management interventions and remote patient monitoring programs addressing leading causes of death and disability in South Dakota's rural communities. The proposal encompasses two components: Component 1 focuses on chronic disease management activities including care coordination, community health worker programs, blood pressure monitoring, cancer screening, diabetes self-management education, and pharmacist-led medication management; Component 2 addresses remote patient monitoring equipment purchases and Electronic Health Record system upgrades. The form mandates applicants identify affected South Dakota counties, document identified needs with local and systems-level data, describe project plans with clear measurable goals and 3-4 specific activities per goal, identify target populations and stakeholders, and establish 3-5 performance metrics with baseline measurements and year four goals. Proposals must be submitted by May 14, 2026, at 5:00 PM CST, with an anticipated award decision on June 19, 2026. The contract term runs from July 1, 2026, through October 30, 2027, with optional renewal for three additional one-year periods.
Eligible applicants include South Dakota clinics, hospitals, pharmacies, dental clinics, tribal health facilities, and Indian Health Services entities, with preference given to organizations in and serving rural and frontier communities. Individual award values are capped at $150,000 per 12-month year, with year one budgets extended to 16 months and capped at $200,000; Component 2 equipment purchases are limited to no more than $50,000 over the four-year funding period. Allowable costs include personnel with salary and fringe benefits capped at a federally approved indirect cost rate of 6.3%, supplies, travel at state per diem rates, remote patient monitoring equipment, and pre-approved conferences and trainings. Unallowable costs include gift cards, food, incentives, research activities, lobbying, out-of-state travel, construction or renovation, and permanent equipment unrelated to remote patient monitoring. The project is 100% funded by the Centers for Medicare & Medicaid Services/Health and Human Services with total financial assistance of $189,477,607.26 across all participants. No incumbent contractors are identified, and applicants must disclose any other Rural Health Transformation funding applied for and potential duplication of efforts. No carry-over funding is permitted, requiring updated budgets and work plans annually.
