Attachment A: RFP #26-09RHT-011 Application Guidance
Description:
The purpose of this funding is to address the leading causes of death and disability in South Dakota’s rural communities by implementing chronic disease management activities. Work will comprise of two components focused on chronic disease evidence-based interventions and remote patient monitoring programs.
Component 1:
Component 1 focuses on the development and implementation of evidence-based chronic disease interventions. Activities must be evidence-based with an emphasis on long-term sustainability and systems change. This may include cross-cutting activities, such as chronic disease care coordination, addressing barriers to health with Community Health Workers or Community Health Representatives, and screening and referrals for social drivers of health; or more condition-specific interventions such as self-measured blood pressure programs, cancer screening reminders, and medication management programs. A list of example activities can be found in Attachment C.
Applicants may propose multiple interventions across chronic conditions.
Component 2:
Component 2 provides funding for the purchase of remote patient monitoring (RPM) equipment and technology. This may include, but is not limited to, blood pressure cuffs, glucometers, and pulse oximeters. This equipment may be provided or lent to eligible patients. This funding is not intended to replace Medicaid, Medicare, or other third-party payer support for the purchase or provision of RPM equipment. Purchases of RPM equipment should be relative to the applicant’s service population and justified within the Attachment B Project Narrative Form.
This funding may also be used to support Electronic Health Record (EHR) upgrades relative to the implementation of RPM programs.
If an applicant chooses to apply for Component 2 funding, there must be a corresponding remote patient monitoring activity in Component 1.
