About this file
This is a Project Narrative Form submission document for the South Dakota Department of Health, Office of Chronic Disease Prevention and Health Promotion, supporting the Rural Health Transformation initiative's Chronic Disease Management Programming RFP #26-09RHT-002. The RFP seeks health organizations—including South Dakota clinics, hospitals, pharmacies, dental clinics, tribal health facilities, and Indian Health Services providers—to implement chronic disease management activities across two components focused on evidence-based interventions and remote monitoring programs. Preference is given to applicants serving rural and frontier communities. The form requires applicants to identify impacted South Dakota counties, provide a needs assessment with local and systems-level data, describe the project scope addressing the identified gap, and establish clear, measurable goals with three to four specific activities per goal for completion within the funding period.
The narrative form mandates applicants describe their target populations, identify stakeholders and community partners, explain how activities create systems-level impact on chronic disease management, and provide evidence-based support for the proposed project approach. For Component 2 applications, a connected Remote Patient Monitoring activity must be included in Component 1. Applicants must submit three to five performance measures with baseline measurements and year four goals, with mandatory measures including hospital readmission rates for individuals with chronic conditions and the number of Remote Patient Monitoring patients reporting controlled blood pressure. All narrative sections are limited to 500 words or fewer, with inquiries directed to Laura Streich at Laura.Streich@state.sd.us using subject line "RFP #26-09RHT-002," and all inquiries and State responses are posted on South Dakota's e-procurement system.

You must include any of the three performance measures listed below if they apply to your proposed activities.
Submit 3-5 additional performance measures to track success throughout the funding period. Include baseline measurements and goals for all performance measures.
Performance Measure Baseline Year 4 Goal
Hospital readmission rates for individuals with chronic conditions.
Number of Remote Patient Monitoring patients reporting a change of their blood pressure to controlled (blood pressure readings being in the normal range <140/90, current medication adherence, and no medication changes needed).
Number of patients engaged in Remote Patient Monitoring.
Sustainability Plan:
Explain how the project’s impact will continue beyond the funding period, such as integration into existing programs, use of other sustainable funding, or community partnerships.
Timeline:
Timeline should include detailed milestones through all four (4) years of the funding period, ending in October 2030. This should provide a clear outline of anticipated progress throughout the funding period.
