About this file
This document is a Proposer Information and Certification Sheet for RFP #S-DASOBO-00014873, issued by a state agency in Oregon. The form requires potential proposers to complete and certify several key compliance statements as part of their bid submission. Proposers must acknowledge understanding of the RFP requirements, agree to contract terms, and commit to performing the scope of work according to specified performance standards.
The certification sheet requires proposers to verify several critical compliance elements, including: non-discrimination in employment practices, tax law compliance, absence of conflicts of interest, independent proposal preparation, and confirmation that the proposer and its employees are not on federal exclusion lists. The document also highlights specific Oregon statutes and regulations, such as ORS 200.055 regarding disadvantaged business enterprises and ORS 279A.112 concerning workplace harassment policies. Proposers must certify the truthfulness of their submission and understand potential legal consequences for false claims under the Oregon False Claims Act.
