About this file
This is a Present/Past Performance Questionnaire template for RFP FA8575-26-R-B004, designed to be completed by government evaluators or contract officers assessing a contractor's historical performance on federal contracts. The questionnaire is classified as sensitive source selection information and must not be provided to offerors. Respondents complete the form by providing their contact information, agency affiliation, and details about the specific contract being evaluated, including contractor name, CAGE code, program title, contract number, contract type, period of performance, and contract dollar values.
The questionnaire employs a six-point rating scale (N/A, Unsatisfactory, Marginal, Satisfactory, Very Good, and Exceptional) to assess contractor performance across ten specific criteria: subcontractor management and coordination; timeliness of deliverables; compliance with contract requirements and performance metrics; inventory control techniques; on-time delivery and quality of deliverables; product/service quality and adherence to quality assurance plans; compliance with military packaging and marking requirements; overall performance satisfaction; warranty response times and corrective actions; and independent performance without excessive customer direction. The questionnaire also includes narrative sections requiring detailed responses regarding project delays, contract terminations or cancellations, disputes or claims, contractor strengths and weaknesses, award recommendations, and value-for-money assessments. Instructions specify that any rating other than "satisfactory" must be supported by documentation; unsupported non-satisfactory ratings will be interpreted as satisfactory.
