DHMO S700B PBC Access+ Dental Plan Schedule of Benefits Summary
This document is a Schedule of Benefits for the S700B-PBC Access+ dental plan, a preferred dental health maintenance organization (DHMO) offering preventive, restorative, orthodontic, and surgical dental services to eligible members in Florida. The plan provides comprehensive coverage across diagnostic and preventive care, general dentistry, endodontics, periodontics, prosthodontics (both removable and fixed), implant-supported prosthetics, and oral surgery procedures. Members have immediate eligibility with no waiting periods or deductibles and receive most diagnostic and preventive services at no charge when utilizing participating in-network providers through the Solstice Benefits network. Out-of-network services are covered at scheduled reimbursement rates to members. The plan accommodates member choice through a provider selection portal at www.SolsticeBenefits.com and offers member services support via telephone at 1.877.760.2247. Specialty services are available through participating orthodontists, oral surgeons, endodontists, periodontists, and pediatric dentists, with members able to self-refer to any participating specialist.
The Schedule of Benefits establishes specific in-network copayment amounts and out-of-network reimbursement rates for hundreds of dental procedure codes organized by service category, with copayments ranging from zero dollars for many preventive services to several hundred dollars for complex restorative and surgical procedures. Additional material and laboratory fees apply to procedures marked with asterisks, including charges for high noble metal (up to $145), titanium (up to $120), noble metal (up to $120), base metal (up to $55), crown laboratory fees (up to $155), and ceramic/porcelain fees (up to $155). The plan includes significant limitations on frequency of services, such as one comprehensive oral evaluation per 36 months, one bitewing X-ray set per 12 months, one prophylaxis or periodontal maintenance per six months, and one occlusal guard per 36 months. Specific restrictions apply to pediatric services including sealants limited to children under 16 on unrestored permanent molars and space maintainers limited to children under age 16. New dentures include one reline within six months, and replacement of crowns and fixed bridges is limited to one replacement every five consecutive years, with additional charges of $30 per unit when crown work exceeds six consecutive units.
