About this file

The document is a Price Schedule Attachment B for RFP 25 8302 DPS Post-Hire Occupational Health Assessments issued by the Orlando Utilities Commission (OUC) in Florida. The solicitation seeks qualified suppliers to provide post-hire occupational health assessments through two service models: mobile onsite services and a brick-and-mortar facility, covering six OUC locations across Orlando, St. Cloud, and Titusville. The procurement encompasses comprehensive medical evaluations including asbestos assessments, respirator medical questionnaires, quantitative fit tests, electrical resistivity tomography, and audiometric testing. The proposal is due on July 16, 2025, at 2:30 PM EST, with a three-year contract term and potential for two additional one-year renewal options.

The pricing schedule demonstrates unit costs for various medical examinations, with the brick-and-mortar services totaling $871,600 for the estimated test quantities. Key pricing provisions include fully burdened rates covering all associated costs, with no additional fees reimbursed beyond the listed prices. Suppliers can maintain these prices for three years, with the option to request price increases not exceeding 3% thereafter. The solicitation allows for multiple supplier awards, emphasizing flexibility in service delivery. Invoicing will be based on actual tests completed, and the pricing schedule serves as an evaluation tool rather than a committed funding amount. The procurement includes strict HIPAA compliance requirements and mandates comprehensive documentation of licensing, insurance, and financial capabilities.
