C.1 SCOPE OF CONTRACT

The project includes road repair and related work on three campground roads and one Forest Road, they are: Forest Glen Campground Road, Hell Gate Campground Road, Scott Flat Campground Road, and Forest Road 1S26, as well as campground spurs and parking areas.

Contractor is responsible for furnishing all materials, labor, equipment, transportation, and incidentals to carry out and complete the work described herein, as shown on the Project Plans, and as otherwise required by the contract. All work shall conform to the Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP-14) and the Forest Service Supplemental Specifications (FSSS). The Sections and Subsections referenced below are the FP-14 and the associated FSSS sections.

C.2 PROJECT LOCATION

From Interstate 5 in Red Bluff, CA, take State Hwy 36 west approximately 75 miles to Hells Gate Campground. Scott Flat Campground Road is immediately accessible from southeasterly from Hells Gate Campground. Forest Glen Campground is west of Hell Gate Campground on Hwy 36 approximately 0.9 miles and on the northerly side of Hwy

36. The turn off to Forest Road 1S26 is west of Forest Glen Campground approximately 700 feet on the southerly side of Hwy 36.

C.5 STATEMENT OF WORK

SCOPE OF WORK

This project is located on the Hayfork Ranger District within the Trinity River Management Unit of the Shasta-Trinity National Forest. The project includes road repair and related work on three campground roads and one Forest Road, they are: Forest Glen Campground Road, Hell Gate Campground Road, Scott Flat Campground Road and Forest Road 1S26, as well as campground spurs and parking areas.

Contractor is responsible for furnishing all materials, labor, equipment, transportation, and incidentals to carry out and complete the work described herein, as shown on the Project Plans, and as otherwise required by the contract. All work shall conform to the Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP-14) and the Forest Service Supplemental Specifications (FSSS). The Sections and Subsections referenced below are the FP-14 and the associated FSSS sections.

BASE CONTRACT WORK

FOREST GLEN CAMPGROUND ROAD. The base contract work includes, but not necessarily limited to:

• 15101A- Mobilization, which shall include mobilization and demobilization to and from the project site, fire plan compliance, equipment cleaning, and incidentals.

• 20401A – Excavation/Embankment Construction for Access Paths:

Clearing and grubbing of access pathway shall be included in this pay item. Cut brush and slash materials shall be uniformly distributed and dispersed into the surrounding vegetation areas.

Excavation and embankment construction activities for the establishment of graded and compacted access paths and in preparation for the placement of aggregate base materials as described herein and elsewhere in the Project Plans and this Contract. Compaction Method 2.

Access path gradients shall not exceed 5% longitudinal grade along the path of travel and shall not exceed a crossfall (transverse to the path of travel) of 2% (1/4-inch vertically in 12 inches horizontally).

• 30316A - Roadway Reconditioning – AC Paved and Aggregate Roads:

Ditch and shoulder reconditioning per 303.03 and 303.04 Roadbed reconditioning and Subsections 303.05 and 303.06, Compaction Method 2 Roadway reconditioning on paved roads shall include clearing of debris and cleaning of paved surfaces before applying tack coat.

Clearing existing culverts, 10 feet upstream and downstream of inlets & outlets, and 4’ either side of drainage path, and culvert free and clear throughout length.

Refer to Project Plan Typical Sections for limits of roadway reconditioning and clearing limits.

• 30317A - Roadway Reconditioning – Campground Spurs & Parking Areas:

Ditch and shoulder reconditioning per 303.03 and 303.04 Roadbed reconditioning per Subsections 303.05 and 303.06. Compaction Method 2 Refer to Project Plan Typical Sections for limits of roadway reconditioning and clearing limits.

• 30104A – Aggregate Base for Campground Spurs, Parking Areas and Access Paths:

Furnishing, placing, spreading, and compacting aggregate base (AB) of the type, gradation, locations, and thickness as shown on the plans. Unless stated otherwise on the plans or in this statement of work, aggregate base shall be equivalent to Caltrans ¾” Class II Aggregate Base and no less than 3” in compacted thickness.

Thicknesses of AB called for on the Project Plans are the measured thickness after in-place compaction (95% min relative compaction) of the AB materials. Compaction Method 2

Aggregate Base to receive a top course of asphalt concrete (AC) shall be treated with emulsified asphalt prime coat per Section 411 (FP-14).

Use of AB for repairing of potholes is considered incidental to preparation of the roadbed for asphalt concrete overlay and no additional compensation will be allowed.

• 40304A – 2” Asphalt Concrete Overlay (with Tack Coat) Furnishing and placing tack coat per Section 412 in advance of asphalt concrete placement.

Furnishing and placing 2 inches asphalt concrete (AC) hot mix as per Caltrans ½-inch

Type B, PG64-10 and per Section 403 and Subsection 703.07.

• 63501A - Temporary Traffic Controls:

Campground access road shall be closed during construction activity. Contractor shall provide all temporary traffic signage, barricades, cones and related equipment for said closures.

Contractor shall notify COR seven (7) calendar days in advance of temporary closures of campground access.

Temporary traffic controls shall be provided in accordance with Section 635.

HELL GATE CAMPGROUND ROAD, the base contract work includes, but not necessarily limited to:

• 15101B- Mobilization: which shall include mobilization and demobilization to and from the project site, fire plan compliance, equipment cleaning, and incidentals.

• 20401B – Excavation/Embankment Construction for Access Paths:

Temporary traffic controls shall be provided in accordance with Section 635.

SCOTT FLAT CAMPGROUND ROAD, the base contract work includes, but not limited to:

• 15101C- Mobilization: which shall include mobilization and demobilization to and from the project site, fire plan compliance, equipment cleaning, and incidentals.

• 20401C – Excavation/Embankment Construction for Access Paths:

Clearing and grubbing of access pathway shall be included in this pay item. Cut brush and slash materials shall be uniformly distributed and dispersed into the surrounding vegetation areas.

Excavation and embankment construction activities for the establishment of graded and compacted access paths and in preparation or the placement of aggregate base materials as described herein and elsewhere in the Project Plans and this Contract. Compaction Method 2.

Access path gradients shall not exceed 5% longitudinal grade along the path of travel and shall not exceed a crossfall (transverse to the path of travel) of 2% (1/4-inch vertically in 12 inches horizontally).

• 20402C – Drainage Excavation/Embankment Construction, Rolling Dip:

Clearing and grubbing of access pathway shall be included in this pay item. Cut brush and slash materials shall be uniformly distributed and dispersed into the surrounding vegetation areas.

Excavation and embankment construction activities for the establishment of the Rolling Dips elsewhere on the Project Plans.

• 21101C – Roadway Obliteration:

Obliterate existing unpaved skid trail/road, skid trail length is approximately 100 feet in length. Obliteration shall be Method 1.

All work shall be per Section 211 unless otherwise specified herein or on the Project Plans.

60201C – Install 18” CMP Culvert:

Furnish and Install Corrugated Metal Pipe Culvert. Pipe, couplings and hardware shall be galvanized steel, 16 gauge min, conforming to Section 707. Length shall be as specified on the Project Plans.

Bedding and installation shall conform to the Project Plans and Section 602.

• 30316C - Roadway Reconditioning – Campground Roads:

Ditch and shoulder reconditioning per 303.03 and 303.04 Roadbed reconditioning and Subsections 303.05 and 303.06.

Compaction Method 2.

Roadway reconditioning on paved roads shall include clearing of debris and cleaning of paved surfaces before applying tack coat.

Clearing existing culverts, 10 feet upstream and downstream of inlets & outlets, and 4’ either side of drainage path, and culvert free and clear throughout length.

Refer to Project Plan Typical Sections for limits of roadway reconditioning and clearing limits.

• 30317C - Roadway Reconditioning – Campground Spurs & Parking Areas:

• 70501C – Install Boulder Delineators:

Furnish and install 250 boulder delineators (rocks) as indicated on the Project Plans

(Sheets 6, 7, & 8).

Boulders shall be of the type, quantity, location, and placement shall be as shown on

Project Plans.

Contractor furnished rock materials for boulders shall conform to Section 705.

Existing boulders for relocation and placement:

o Approximately 225 (see Scott Flat plan for approximately location) o Approximately 25 at Hell Gate, within parking area Sta 0+87

Temporary traffic controls shall be provided in accordance with Section 635. Traffic interruptions, such as temporary road closures to through traffic, when authorized by the CO, shall not be allowed for more than 20 minutes.

OPTION WORK

15301 - Contractor Quality Control, the option work includes, but not necessarily limited to:

Contractor shall prepare and implement a quality control plan in accordance with Section

153.

A part time quality control manager (QCM) shall be included as described in Subsection

153.02(a)(2).

• Part-time on-site QCM shall have a minimum of two years of experience in roadway construction, inspection, quality control and materials testing. Emphasis is on grading, roadway preparation, compaction, paving and asphalt material compliance.

Quality control preparation and implementation shall be for all construction work under the contract.

The Quality Control Plan (QCP) shall be reviewed and approved by the Government through the submittal review process prior to issuance of the Notice to Proceed. This does not preclude from FAR 52.246-12 having a Quality Control Plan.

PAYMENT

Payment for all work shall be as provided in the schedule of items in the contract, whether directly or indirectly, and shall include full compensation for all work under this contract.

PROJECT LOCATION

From Interstate 5 in Red Bluff, CA, take State Hwy 36 west approximately 75 miles to Hells Gate Campground. Scott Flat Campground Road is immediately accessible from southeasterly from Hells Gate Campground. Forest Glen Campground is west of Hell Gate Campground on Hwy 36 approximately 0.9 miles and on the northerly side of Hwy 36. The turn off to Forest Road 1S26 is west of Forest Glen Campground approximately 700 feet on the southerly side of Hwy 36.

SPECIFICATIONS APPLICABLE TO THIS INDIVIDUAL PROJECT

FHWA Standard Specification 2014 (FP-14) applies.

Forest Service Supplemental Specifications (FSSS) as attached to the contract.

SITE SECURITY

All security aspects associated with contractor owned equipment and materials are not the responsibility of the Government. The Government assumes no liability whatsoever for Contractor use of designated staging/laydown/overnight area. Contractor is solely responsible for the security of his/her possessions related to this project.

PROTECTION OF EXISTING IMPROVEMENTS

Existing pavement, utilities, landscaping, and other improvements to remain shall be protected by the Contractor. The Contractor shall be responsible for repairing any damage to existing facilities and improvements as a result of work done yet outside the project scope of work.

USE OF UTILITIES

Potable water may be available on site and shall be coordinated with the Contracting Officer’s Representative (COR) for access. Sanitary services shall be provided by contractor at no additional cost to the government. Power is not available at the site.

Water Drafting Sources: Authorized for Contract Work Only Drafting Sites: Trinity River where accessible from Hell Gate Campground or from Scott

Flat Campground Road river access.

• Fish screens shall be used on drafting intakes.

• Contractor to submit fish screen system to COR for approval prior to use.

Contractor to protect surrounding vegetation and access ways from damage due to equipment engaged in drafting activities.

Disturbed areas due to drafting activities shall be restored to pre-project conditions.

Water access, use and drafting restrictions are provided for the convenience of the contractor and at no additional cost to the government.

DISPOSAL

All construction waste material shall be properly and legally disposed of off government land.

TIME OF COMPLETION

Work to be completed within 160 calendar days following Notice to Proceed (NTP) from the Contracting Officer.

(1) The function, operation, or use of which involves intelligence activities; involves cryptologic activities related to national security; involves command and control of military forces; involves equipment that is an integral part of a weapon or weapons system; or is critical to the direct fulfillment of military or intelligence missions, but does not include a system that is to be used for routine administrative and business applications (including payroll, finance, logistics, and personnel management applications); or

(2) Is protected at all times by procedures established for information that have been specifically authorized under criteria established by an Executive order or an Act of Congress to be kept classified in the interest of national defense or foreign policy.

Sensitive compartmented information means classified information concerning or derived from intelligence sources, methods, or analytical processes, which is required to be handled within formal access control systems established by the Director of National Intelligence.

Sensitive compartmented information system means a national security system authorized to process or store sensitive compartmented information.

Source means a non-Federal supplier, or potential supplier, of products or services, at any tier.

(b) Prohibition.

(1) Unless an applicable waiver has been issued by the issuing official, Contractors shall not provide or use as part of the performance of the contract any covered article, or any products or services produced or provided by a source, if the covered article or the source is prohibited by an applicable FASCSA orders as follows:

(i) For solicitations and contracts awarded by a Department of Defense contracting office, DoD FASCSA orders apply.

(ii) For all other solicitations and contracts DHS FASCSA orders apply.

(2) The Contractor shall search for the phrase “FASCSA order” in the System for Award

Management (SAM) at https://www.sam.gov to locate applicable FASCSA orders identified in paragraph (b)(1).

(3) The Government may identify in the solicitation additional FASCSA orders that are not in SAM, which are effective and apply to the solicitation and resultant contract.

(4) A FASCSA order issued after the date of solicitation applies to this contract only if added by an amendment to the solicitation or modification to the contract (see FAR 4.2304(c)).

However, see paragraph (c) of this clause.

(5)

(i) If the contractor wishes to ask for a waiver of the requirements of a new FASCSA order being applied through modification, then the Contractor shall disclose the following:

(A) Name of the product or service provided to the Government;

(B) Name of the covered article or source subject to a FASCSA order;

(C) If applicable, name of the vendor, including the Commercial and Government

Entity code and unique entity identifier (if known), that supplied or supplies the covered article or the product or service to the Offeror;

(D) Brand;

(E) Model number (original equipment manufacturer number, manufacturer part number, or wholesaler number);

(F) Item description;

(G) Reason why the applicable covered article or the product or service is being provided or used;

(ii) Executive agency review of disclosures. The contracting officer will review disclosures provided in paragraph (b)(5)(i) to determine if any waiver is warranted. A contracting https://www.sam.gov/ https://www.acquisition.gov/far/part-4#FAR_4_2304 officer may choose not to pursue a waiver for covered articles or sources otherwise covered by a FASCSA order and to instead pursue other appropriate action.

(c) Notice and reporting requirement.

(1) During contract performance, the Contractor shall review SAM.gov at least once every three months, or as advised by the Contracting Officer, to check for covered articles subject to FASCSA order(s), or for products or services produced by a source subject to FASCSA order(s) not currently identified under paragraph (b) of this clause.

(2) If the Contractor identifies a new FASCSA order(s) that could impact their supply chain, then the Contractor shall conduct a reasonable inquiry to identify whether a covered article or product or service…
